A severe Stahl’s ear malformation has a much more severe bend to the superior antihelical crus. Your bend looks pretty gentle and would probably be amenable to traditional otoplasty suture techniques. You would benefit from “scapha” reduction to reduce the height of the ear, which also may improve the “roll” of the helix.
Sorry for a lot of technical terms. Talk with an experienced otoplasty surgeon.

The quest for perfect symmetry is sometimes frustrating. I do see the differences that you note. There appears to be more fullness and skin over the right (your right, the eye on the left hand side of the photo) upper eyelid region. Asymmetrical incisions may have been made initially to compensate for any pre-existing asymmetry.
To further provide balance to your results, further skin excision from the right upper eyelid, with a little bit more fat resection, might help…

I’m really sorry to hear about your accident. It sounds and looks  pretty bad. You do have both a jaw fracture as well as a cheek bone fracture.  I would consider getting perhaps a second opinion. The jaw fracture at the very least should be repaired.

It is called a pre auricular pit. These pits are a congenital remnant from the process that was involved in the creation of your ear while you were a fetus. The pit itself likely leads into a little dead-end tunnel that is lined with the same type of cells that make up the outer skin. You do have a slight anti helical fold in both ears. Accentuating the fold (as is typically done in a regular otoplasty) would not improve the “long” appearance of the ear. There is a lot of distance between your anti helical fold and the helical rim of the ear. The area between the anti helical fold and the rim is called the scapha. You have what appears to be slight excess of the scapha. The scapha is the space between the anti helical fold and the rim of the ear. This can be reduced in size by removing a crescent shaped piece of cartilage through an incision hidden in the rim of the ear. Ear pinning and scapha reduction could potentially be done simultaneously.

It appears that you may have an inclusion cyst or milia, which is not uncommon. This occurs as skin cells inadvertently are buried by the suturing process, which is why it is on the incision line. This can be treated simply by completely unroofing the cyst, allowing the contents to empty. This can be done fairly easily in the office. It might take one or a few treatments. See your original surgeon.

Yes, the top of the ear can be reduced in size so that the entire ear is not as “tall” and the upper border contour can be re shaped. It would likely involve an incision just inside the edge of your ear inside the helix, hiding the incision. The cartilage between the helix and the antihelix (scapha) can be partially trimmed to reduce the height of the ear. This is not the standard otoplasty that most people request, but can be done.

Looks like a difficult problem. When I harvest auricular cartilage for rhinoplasty, I try to avoid getting too close to the antihelical fold, reducing distortions of the ear. It appears that there is a lack of cochal bowl support because of cartilage resection. In addition, there appears to be relative excessive skin as a result of cartilage removal and subsequent distortion. This could be done, although I would have to see a picture of your ears to understand their “pointyness.” The outer rim of the ear, called the helix, can be somewhat reshaped by reshaping the scapha portion of the ear. The scapha comprises a portion of the ear just inside the arc that forms the helix. Just imagine if you had to reshape a square pizza. You could take off the outer crust (helix), change the inside remaining shape of the pizza from Square to round, and then re-drape the crust….

There are some options. You can use Botox to relax the depressor muscle of the lower lip. This would reduce the amount of lower teeth show when you smile. Botox of course is not permanent. There are permanent solutions which include surgically dividing the depressor muscles of the lower lip. However I would think Botox is easy enough to try out first.

Often monofilament sutures are used because they slide through tissue easier, and perhaps are less prone to infection. Absolutely, there are ways to alter the size if ears beside the “standard” otoplasty procedure. Typical otoplasty is designed to accentuate the anti-helical fold to bring the upper pole of the ear closer to the head, or reduce a prominent concha to bring in the mid portion of the ear.
