The dorsum can be approached with either an internal or external rhinoplasty for both primary and revision surgeries. Onlays can be removed, reshaped and replaced. There are times when the scar tissue will have filled in the area around the graft and a much smaller or no graft may be necessary at all. Surgery can attempt to correct a ‘sensation’ but there can be no guarantees made that any surgery will get rid of a nasal sensation or pain. A spreader improves the internal valve area and it looks like yours is pretty open. There will always be resistance in the valve area and narrowing with inward breathing.

If the upper jaw is OK then you can have your septoplasty and it will not affect your lower jaw. You may want to have both consults before you do anything to make sure that you have a good management plan before your start.

Your septoplasty may have partially improved your breathing, but, if your nose is twisted and the bones have collapsed from a fracture then that may be obstructing your breathing as well. You have internal and external valves inside your nose that sound like they may be compromised as well. If you have a functional breathing issue from a fracture, your insurance should help.

If your nose feels dry you may have had your inferior turbinates reduced or removed. There is a syndrome called “dry nose syndrome” that will cause you to feel obstructed even if your nose is wide open. Decongestants like pseudoephedrine are the same medication used in crystal meth. (See Breaking Bad). Amoxicillin can cause a false positive result for cocaine. They probably won’t but you can always let the testing company know before the results come out or after. See the website below.

Your bleeding and oozing is normal especially for the first week. Your nose looks swollen and looks like you have packing which can also contribute to frontal drainage. You are still very early and need to be patient. When your surgeon cleans out your nose and removes any packing you should feel better.

The largest portion of swelling of the nose will be better within a couple of weeks after surgery. The remainder of the swelling can take many months to improve. Most people are breathing better than before surgery within a couple of weeks. You should see your surgeon. You always have an option of a second opinion should you not recover as rapidly as you expect.
