Is the photo before any surgery or after a rhinoplasty?

Turning down the nasal tip(derotating the tip) does require a full understanding of nasal anatomy as well as cartilage graft material--septum or ear in order to make the nose architecturally stable over the long term.

With experience it is a highly predicable and aesthetic surgery.

Sometimes I will use temporary filler to show the patient how their nose will look after surgery.

Skeletal studies demonstrate that the aging process is one of bone and fat volume loss.

There are two separate issues: aesthetics and function.
Aesthetically you have a strong masculine nose, slightly large, however against a background of weak(sunken) cheeks, temple and forehead. These issues will all progress with age.

Yes, quite easily.
An alaplasty can reduce the size of the nostrils and or their shape including elevating the insertion into the cheek
Your nasal profile is very attractive; be careful not to change other aesthetic aspects for this one view

Are you referring to the subtle "upside down 'V" at the top of your nasal tip?
If so it's the upper edge of your nasal cartilages or the skin memory of where the upper edge of your cartilages used to be..................
Depending on which of the two possibilities it turn out to be will determine the best course of treatment.

The answer is an amalgamation of numerous factors including:
1. Surgeon preference
2. Anesthesia availability---some centers are not approved for general, only sedation

Personally I prefer and use general because:
1. Blood pressure is controlled and stable minimizing swelling and bruising
2. The anesthesia tube protects blood from leaking into the patient's throat during surgery, which can be swallowed leading to nausea and possible vomiting or into the airway

It would be very helpful to see your before photos(same view)
I would suggest not reversing the surgery as it may accelerate midface aging because your periosteal attachments have been released
Instead i would suggest (permanent) fat grafting to the lower and lateral cheek making the cheek prominence look less localized

It could be nasal tip skin quality/quantity/fascial circulation/etc.
Most likely this scenario sounds like poor tip skin support

Because your skin is thin you are able to see the to nasal tip cartilages---like two "McDonald's arches"---you are seeing the space between the arches.
The easiest remedy is a filler---
