Look at their before and afters.  IMO rhinoplasty surgery is a sub-specialty onto itself.  Neither plastic surgeons nor ENT's have a monopoly on good results. Ideally you want to find someone who is a rhinoplasty specialist and who does mostly rhinoplasty surgeries. That person could be a board-certified facial plastic surgeon or plastic surgeon.

Rim grafting can feel quite natural when typical thin septal or ear cartilage grafts are used.  The use of rib grafts or thicker septal cartilages in the alar rim can result in sometimes palpable or stiff alar rim.  It also depends on the size and orientation of your alar cartilages, skin thickness, and other factors. Based on the pics you are probably good candidate for buccal fat removal.  However the real test would be for a surgeon to examine you and feel the cheeks and your bone structure to figure out how much of the cheek fullness may be due to the deep buccal fat vs other factors.  Buccal fat removal should be considered carefully, since once the fat is removed it can't be replaced.  In the right patient, it can add subtle elegance to the face.  Best to you, @drjohnvartanian

Technically you could have the buccal fat removal a month after rhinoplasty.  Having said that, I would recommend waiting 2-3 months so that any rhinoplasty related facial swelling settles down and your surgeon can use better judgement on how much buccal fat to remove.  I perform a large numbers of buccal fat removals under local anesthesia and patients tolerate it very comfortably.   All the best to you, @drjohnvartanian

You are good rhinoplasty candidate.  I would clearly communicate with your surgeon your aesthetic goals. Your nose is also crooked and will need to straightened. One important thing to keep in mind...overly narrowed noses may result in permanent and serious nasal breathing problems.  As such, you need to figure out with your surgeon a nice in-between size reduction, other functional procedures (like septoplasty) so that you get the prettier nose you want + preserve  your nasal breathing fun...

Rasping a medium to large hump will get you a profile improvement, but in almost all cases the bony part of the nose will look wider than ideal. In very small humps, rasping alone may be used...but, most hump reductions need lateral osteotomies in order to close the gap between the bones (open roof), narrow the nasal bones, and give a better aesthetic look to the nose on frontal view. In some revision rhinoplasty cases cartilage needs to be added to the nose for support and/or looks. Correcting a pinched tip may be one such example. We generally get extra cartilage from the following places (in order of ease and general preference): Septum, ears, patient's own rib cartilage, and cadaveric rib cartilage. Hi,  6-months after rhinoplasty your nose should be stable enough not to be harmed by extractions or a facial. So it is highly unlikely that the facial did any harm or contributed to your nose getting crooked.  Most likely the swelling is subsiding, now revealing some of the underlying imperfections.

Cold compresses in the first 48 hours are helpful in reducing swelling and discomfort.  Beyond that period the nature of compresses probably make little to no difference to you recovery.  Warm compresses can sometimes be useful in breaking clots, but highly doubtful if it actually makes any difference.  So, don't stress too much over the compresses.

Dear Morocha10,  The Fraxel repair cannot be used for dark skin as it may destroy pigments in the skin and cause lighter scar-like areas (hypopigmentation).  Also, the possible heat injury from the laser may actually make your keloid worse.  So no to the Fraxel repair or any other co2-based laser for you.
