I don’t know your specific anatomy, but I would never use 600-700cc implants at the same time as a lift. These combined procedures are very difficult to do together especially large lifts or large implants.

Patients fall into 1 of 3 categories:

1) small lift needed, definitely need more volume–> implant first (perhaps a shaped implant to get better lower pole fill and rotation of nipple upwards); then a lift later if they need it.

On the front shot, I can’t completely see your chin, but it appears that you have some facial asymmetry, i.e. your chin points a little to the left. Don’t worry– we all do. I always look at your bite and ask if you ever had braces. Our nose is like the roof on the house. The roof can only be perfectly straight if the foundation is, and that foundation starts with how your teeth come together (your bite).

You could do a fairly subtle revision of the tip. It should be done “open.” You have a beautiful well proportioned face. Overreduction of the tip or failure to adqeuately support the tip would the biggest possible pitfalls with you. Ask if your surgeon has VECTRA 3D imaging. It would really help as a communication tool.

Vectra is an excellent tool– particularly a communication tool with the patient to discuss how you wish to look. However, there are a few caveats:

1) I don’t find the sizes to be predictive. Rely on your surgeon’s judgement in the operating room based on him or her understanding your wishes and using his or her experience rather than settling on a specific size preoperatively.

“Gummy bear” implants are term used to describe a more cohesive silicone implant– but they are still silicone. Silicone is FDA approved and there are no autoimmune diseases which have been linked to their use.

These more cohesive implants have benefits for certain patients. They typically offer less wrinkling, but if you decide to used a shaped implant, they can rotate.

Septoplasty does not leave any external scars. Rhinoplasty (if done open or with nostril excisions will leave scars. One of the classic issues with transaxillary augmentation is not getting the implant position correct. This can be improved by using a small camera called a scope to make sure your pocket is correct in surgery rather than doing it blindly. If the muscle is dissected well enough, they will usually drop some. If the muscle is under-released, it can become worse.

In my experience, rotation or malposition is the number one problem with these implants which can otherwise be an excellent option depending on your anatomy and outcome desires.
