6 weeks post op, I’m bottoming out. Any suggestions?

Sep 15, 2015
Hi and thanks for your question and sharing pictures. Like others have said it is way to early to judge the result. Looking at the picture I don’t see obvious signs of bottoming out – a true lateral would help but your nipple position seems good from the 2 pictures provided. What you may have noticed is the implants settling into their pockets, but from your pictures they look to me to be in a good position currently. You won’t like the answer on how to correct bottoming out if it does…
Can a plastic surgeon show me how I would look before I got the surgery? Is there a program wh
Software not reliable…
Sep 15, 2015
Hi and thanks for your question. Unfortunately there is no medical imaging software that does this well. The only reliable change in the nose that can be mimicked fairly close to what you can expect is a lateral view of dorsal nasal hump reduction (and possibly dangling columella correction) – and this is easily done on paper with a black pen. Otherwise the software in my experience is only good to demonstrate very gross changes to nasal shape and how this may affect the overall appearance…

Sep 11, 2015
Hi amborcky85 and thanks for the question. I hope you are healing and feeling well. There is no advantage of performing fills once versus twice weekly other than you can get filled faster to your “final volume” twice weekly. It also means more frequent trips for you to your plastic surgeon as a trade off. If you need radiation therapy after mastectomy occasionally it is necessary to fill twice weekly to get to your desired size so as not to delay radiation (which usually starts 4-6 weeks…
After 2 previous septoplasty operations would it be considered as a “revision” surgery if I ha
Depends…
Sep 11, 2015
Thanks for your question. If the septoplasty procedures were performed from inside the nose then I would not personally consider this a secondary rhinoplasty. Some septoplasty procedures are performed from a “top down” or open approach which is the same approach to rhinoplasty (you’d have a scar across your columella) and then there could be scar tissue making a rhinoplasty seem more like a secondary procedure. As mentioned above if cartilage is needed this may no longer be present and may…

Any Breast Changes?
Sep 10, 2015
Thanks for your question. If the sensation was normal and all of a sudden is changing this could be due to multiple factors, certainly one is the implants. If you are developing capsular contracture around the implants (scar tissue tightening) this may be also pulling on the soft tissues affecting nerve transmission for example. Because of this I would go and have the implants evaluated by the surgeon who placed them or another board certified plastic surgeon.
Is it OK to take Percocet?
Not being silly
Sep 10, 2015
Thanks for your question. You are not being silly. This is an operation that can cause significant soreness in the first few days and it is perfectly reasonable to take percocet (oxycodone/tylenol combo) to control pain. I have patients take tylenol as needed and use oxycodone for moderate to severe pain in my own practice routinely after this surgery. There is stigma around these medications for sure, but there is a very low chance of becoming addicted when used as prescribed following…
What does this look like?
Multiple causes…
Sep 10, 2015
Thanks for your question. While none of us are there or able to examine you there can be multiple causes of this and there may be several of them happening at the same time (external factors, surgical factors, patient factors, and sometimes “things just happen”). The picture is very narrow so its difficult to say if you have an infection – this would involve redness that is spreading further than what I can see in the picture. This can be breakdown from a spitting stitch or dissolvable…
Will rhinoplasty be helpful in my case or injecting filler like Radiesse?
Fillers for nose
Sep 10, 2015
I apologize that I have no idea the cost of rhinoplasty in India…I would seriously caution you against using permanent fillers in the nose. You may not like the result, and you can’t really remove it easily. Plus with the thin skin overlying the nose there will be irregularities that you probably can’t reverse. If you are thinking of a filler, use a non-permanent (hyaluronic acid based) filler so you can see if you like the result. I have also seen complications from permanent fillers…
How long does it take for nerves to grow back to normal?
Nasal discomfort after Rhinoplasty
Sep 10, 2015
It can take nerves 12 months (or longer) to completely heal following this procedure. Many patients will have normal sensation by 3 months, some take 6 months, and others 9-12 months. If it was an open rhinoplasty then it may take them longer, especially since you had a septoplasty (more dissection). If you didn’t have the burning sensation initially and then it started it could be due to several things: Neuroma (the nerves are healing but some got stuck in scar tissue or can’t “find their…
Doctor doesn’t show pictures. Should I be concerned?
Doctor Doesn’t Show Pictures
Sep 3, 2014
There can be several explanations. First as mentioned I would make sure the physician is a trained plastic surgeon. We get consent from patients to show their pictures to other patients in my practice. This can simply be in a photo book that we show patients while they are having a consultation, and we obtain a separate consent to post images online. Some patients refuse to have their pictures shown even to other prospective patients in our practice, so we don’t show them. This surgeon may…
Do I need a revision?
Do I need a revision?

I also see the volume asymmetry. Was the right breast larger to start off with? If so that is your answer. Also depends on how long ago your surgery was as the implants can drop at different rates. Wait at least 9 months, preferably a bit longer and they may level out. Looks like scars are well healed, no rippling, and top part of implants are level so they are positioned well. I personally would not revise this unless you absolutely can’t live with it as sometimes a revision of an asymmetry…
What size implant would I be able to have?
What size implant would I be able to have?
Sep 2, 2014
Realistic expectations are important. It’s also important to keep in mind that cup size is also highly variable between manufacturers of bras and highly dependent on your chest wall circumference (so a DD in company A may only be a C or D in company B’s bra). So the difference of getting to a DD cup on smaller and larger framed individuals can require drastically different sized implants, and then the bra you buy may may have an ‘inconsistent’ size. It is difficult to tell what maximum size…
