The nose will swell a little from insertion of a columellar strut. I assume the insertion would be through a small incision on the columella. If the columellar strut is inserted by "opening the nose up" via a standard open rhinoplasty approach then the swelling will be much greater and for up to a year.
Can I get away with only implants?

Because you have ptosis (drooping) and the nipple/areolar are at the inferior border of your breast you would be best served with a lift as well. An implant alone would not produce an aesthetically pleasing result.

Extrusion of sutures is not uncommon. They can be a sourse of local infection and in rare cases progress to more serious infection. It should be removed by your surgeon. Good luck.

I agree with the other comments that it is difficult to answer your question definitively without additional photos particularly a side view. However, generally speaking augmenting the dorsum does give the appearance of a more narrow dorsum on frontal view. Hope this helps.

In my opinion your best option is a breast lift which will reshape the breasts, reposition the nipple/areolar complex and lift the breasts. If you tend to form keloids or hypertrophic scars, then aggressive scar management during the postoperative period will be important including silicone gel sheeting. I agree with other comment that breast lift scars can be quite visible especially if you form poor scars.

How big you will be after placing 800cc implants depends in part on how on your height, weight, breast width, torso width and other factors. However, 800 cc implant will increase you by a minimum of two cup sizes and more likely 3+ cup sizes. 800cc implants are obviously quite large and not recommended in the average patient for breast augmentation in my opinion. Although it is difficult to offer a definitive opinion based solely on pictures, I think it is very likely that you would require a breast lift for the best result. Whether or not a breast lift is necessary is largely determined by nipple position. If the nipple is below your inframammary fold or pointing down, then usually a breast lift is the best choice. I would combine that with an implant to restore volume.

It is way too early to make an accurate assessment of your nose. Swelling, distortion and asymmetry are not uncommon during the early postoperative period. I advise my patients not to critically evaluate their noses for several weeks after surgery. It will take months for the swelling to resolve and up to a year to see the final result. Be patient. Discuss your concerns with your surgeon.

Typically we determine whether a breast lift is necessary by the position of the nipple relativel to the inframammary fold (breast crease). If the nipple is below the inframammary fold or pointing down then you would benefit from a lift. Based on the picture that you have sent, I believe you would be best served by a breast lift (vertical or inverted T approach). A modest implant would provide improved superior pole fill.

Although it is true that the definition created at the time of rhinoplasty can be masked somewhat by thick skin, your nose can definitely be made more narrow and overall smaller. Cartilage grafts could be considered to improve tip definition.
