In my experience, a relatively small percentage of patients even report joint pains and when they do it is mild (not even enough to motivate them to take ibuprofen) and resolves 1-2 months after starting the isotretinoin.

You are on a good "topical" (creams/gels) regimen but may benefit from oral options in addition to the tretinoin and Epiduo…

I would continue seeing your dermatologist to provide feedback on response so that he/she can make adjustments as needed… remember patience, it can take ~1-2 months for a change in treatment to start "kicking in", until then your acne is naturally going to have some ups and downs

I would recommend you start seeing a local dermatologist to discuss options… the key to controlling acne is consistent use of medicines because they take several weeks to start working and occasionally need to be adjusted to maintain control until you "outgrow" your acne.

"retinoid" type creams, oral or cream/gel antibiotics and anti-inflammatory benzoyl peroxide acne washes/creams are often helpful AND best used in combination with eachother

Irritant Reaction:

1% hydrocortisone ointment (OTC) 2-3x/day for 3-4 days, gentle soap like Dove, avoid excessive sun, would expect recovery within 4-7 days… if not then be seen by a dermatologist

I agree, a local dermatologist will certainly be able to help you.

Assuming it is acne that you are dealing with, I think a good place to start is with a daily benzoyl peroxide wash (over-the-counter is OK) and a leave on "retinoid" type cream (such as tretinoin or adapalene) which requires a prescription.

Initially this combination may cause some dryness (use a moisturizer as needed) but give it at least 2 months to start working.
