I am a smoker and would like to know how long in advance do I need to quit before surgery.
smoking
Sep 6, 2016
Thank you for your question and concern. I typically ask patients to quit 1 month prior to surgery. However, I have had patients who just can’t quit. I have a serious sitdown talk with them letting them know the risk they take by continuing to smoke. The inherent risk is skin tissue loss after the surgery. This is because smoking reduces the amount of blood vessels to the skin and when one does a facelift, you’re already lifting the skin and devitalizing the blood vessels…
Can the ball on my nose be reduced?
bulbous tip
Sep 6, 2016
Thank you for your post and concern. The lower third of the nose is comprised of cartilage in the shape of an “M.” You were born with a strong lower cartilage and a lot of that has to do with genetics. Reshaping of the cartilage in a conservative fashion can give you more tip definition and a softer appearance but most importantly, a natural looking nose. Also skin thickness plays a keyrole with healing and can hide small defects of the cartilage after surgery if you have thicker skin. Hope…
Facelift scarring; what would be the best thing for me to do?
male facelift
Sep 6, 2016
Thank you for your question and concern. I also wanted to add that to make scars less noticeable, typically an incision is ‘beveled’ along the hairline so that the hair grows through the scar and less conspicuous. Hope your facelift went well!Dr. Miguel
My previous rhinoplasty questions – I am nearly 4 YEARS post surgery! How much will a revision cos
revision costs
Sep 6, 2016
Thank you for your question and your concern. regarding your question, revision rhinoplasty does vary from surgeon to surgeon. realistically, revision rhinoplasty tends to be a tougher and longer procedure compared to your first time because of scar tissue and rebuilding a nose. anticipate anywhere from $5-7K realistically for surgeon fee and facility charges. Hope this gives you a good idea of a range. Good luck!Dr. Miguel
Second surgery and sepal perforation has gotten larger. Any suggestions?
septal perforation
Apr 5, 2016
Thank you for your post and Im sorry to hear what you’re going through. Unfortunately septal perforations are an unintended consequence of nasal surgery, even in the best hands. However 1 cm is within the average range at this point and hopefully shouldn’t get larger. however, more surgery to the area can result in a larger perforations. Let your nose heal and most important for you will be to keep the inside of your nose moist. Nasal hygiene will be important and using a gentle nasal…
Will my facelift be canceled due to recent illness?
diverticulitis
Apr 5, 2016
Thank you for your post and I’m sorry about your current situation. Diverticulitis is a serious condition however discuss it with your surgeon. Sometimes being already treated-or in the process of treatment with clinical improvement won’t cancel surgery. Also ask if you are being put under general anesthetic or will your surgery be under twilight where you’re still breathing on your own without a tube. That can also make a difference because being put under general anesthetic has more risk…
What can I do about painful stitches around the back of my ears after having a facelift 4 weeks ag
stitches
Apr 5, 2016
Thank you for your post and concern. To add to the other answers, yes at 4 weeks, sutures should be removed, however, what you might be feeling are scabs rather than stitches. Have your surgeon look at them. Also not uncommon to have most pain behind the ears because this can be an area of high tension. Hope this helps!
My lower earlobe is protruding and I want to know the best way to look like it’s not sticking out?
ear reduction
Apr 5, 2016
Thank you for your post and question! an ear lobe reduction is a local procedure where excess skin is removed and an incision is hidden along natural crease lines. Procedure takes typically an hour and you go home the same day. you look like a good candidate for that. hope this helps!
Cheek swelling after open rhinoplasty?
swelling
Apr 5, 2016
Hello and thank you for your question and post. Swelling around the cheeks/face is quite normal and worst 3-4 days after surgery. This is the worst it’ll get. Especially if they had to break the sides of your nose -which I assume – b/c you have a plastic splint. Hang in there! Also, ice at this point won’t do too much. Hope this helps and good luck!
Will my tip get smaller? Are steroids OK to use?
revision
Apr 1, 2016
Thank you for your post and question. At one year, this is typically your final product and no steroid injection will get your nasal tip to a point that you would like it. From this angle, it appears that your lower lateral cartilage might be to blame. Consider a revision rhinoplasty and getting another opinion from somewhere else. Seek out a physician who only focuses on facial plastic surgery and board certified as they only focus on facial procedures. Hope this helps and good luck!
Should I be worried inside ear is swollen?
1 week postop
Mar 18, 2016
Thank you for your post. It is quite normal to have swelling in that area of the bowl you are referring to. Sometimes people may feel like their hearing might be decreased after the procedure. Being only 8 days postop, you are expected to be swollen. Give it a good 2-3 weeks after the surgery to start seeing that swelling go down. Hope this helps!Dr. Miguel
Is this polly beak deformity?
pollybeak definition
Mar 18, 2016
Thank you for your question and sharing your pictures. There are many reasons for having a pollybeak however the most common are:1.scar formation in that nasal tip2. inadequate take down of cartilage from septum3. over resection of the nasal bones or lower cartilages. From your pictures, it appears you may have that pollybeak deformity, given you are also more predisposed because of your thick oily skin. Typically, conservative treatment is the first course of action. Things such as…
What is the best choice to fill up dead skin area of nose?
nasal tip skin
Mar 18, 2016
Thank you for your post and sharing your pictures. I am sorry to hear this happened to you. Unfortunately, in revision rhinoplasty, there are numerous scars and adhesions one must go through to work on the nose again. The unfortunate consequence is re-violating the blood supply to the skin of the nose when doing a rhinoplasty. The columella(nasal tip) is an extremely small and thin area of skin that is very sensitive to blood supply and this is what most likely happened to your nasal tip….
How swollen am I at 6 weeks?
swelling after rhinoplasty
Mar 18, 2016
Thank you for your post. At about 6 weeks, it is very common to have a good amount of swelling after a rhinoplasty. I typically tell my patients that the swelling can persist a good 3-6 months and that the nasal tip can almost take up to 9 months for the swelling to go down. Your nose will continue to change and evolve and typically one’s final product really won’t be until a year out. Hang in there and hope this helps!Dr. Miguel
I don’t have nose cartilage (left side)
nose cartilage
Mar 15, 2016
Hello and thank you for your post and sharing your picture. What I see from your picture is a loss of support on the left side of your nose. What might’ve happened is that they took out too much cartilage on the left side when compared to the left. Another result from the drooping is it has made your nostrils asymmetrical. The most important question is how much cartilage you have left in your septum(middle dividing wall inside your nose). If there isn’t enough cartilage, a surgeon can…
