Patients with concerns like these are great candidates for a deep neck lift through a single incision under the chin. The deeper tissues of the neck, including fat, muscle, and salivary glands, can drop even at a young age. Also consider a chin implant, which can lengthen the jawline and make the neck angle look more crisp!

I see many young patients who say “droopy” or “saggy.” If you’ve had recent weight loss or are thin build to begin with, a subtle volumizer would be a great option. At this point, consider PLLA (Sculptra) to help you with your concern. Likely you’ll need more than one treatment, and it takes a while to see measurable results. Good news is it can last a long time and your skin looks even better!

Suture placement is all about tension. When we remove skin, we make sure not to remove too much; any gap in the skin when you close with stitches can lead to tension and poor scarring.Surgically, it’s about repositioning your tissues with a deep plane facelift and necklift to help the saggy neck and jowls, as well as cheeks. Upper blepharoplasty +/- a temporal or lateral browlift would really make your eyes shine! For your skin, CO2 laser would help with your skin quality.

Enjoy your beauty in your youth! Re: your upper face, your brow position is not low. Perhaps your tox dose is not optimized, or you metabolize it faster than avg.For your fine lines, there is no one and done treatment – good home skincare, some chemical peel or laser, nothing too intense.For your neck lines, tox and/or hyperdiluted Radiesse treatment with subcision of neck lines (again not a one and done treatment) would be a great place to start.

I am assuming that you have healed more or less completely after facelift surgery.While surgery addresses deeper anatomy to reposition your tissues to a more youthful position, the skin and superficial tissues may benefit from adjunct procedures before and/or after your operation. Subcision with injection of biostimulators after surgery (wait at least 6 months after surgery) can help build collagen in the dermis and superficial tissues if skin laxity is the issue.

It is likely that the issues lie in the deep neck structures behind the platysma muscle. These include the fat behind the platysma, digastric muscles, and submandibular glands, which can be noticeable if surgery was performed only on the platysma and superficial fat. For people with skinnier necks, it can be even more noticeable.

Men, esp with changing cheek volume, are great candidates for extended deep plane facelift with necklift. There is a good chance that the upper cheek volume would be much improved even without fat transfer. Deep plane surgery is the best way to address the midface, and it allows smoother rejuvenation along the lower face and neck as well.
Do I have pseudo “Double Eyelids”?

I see many patients who have this same concern. The anatomy of your eyelid skin, fat, and levator can change over time. The levator is somewhat attached to the eyelid skin to make that crease. It’s not a super strong crease or “double eyelid,” but it looks nice and natural.
Can you perform incisional double eyelid surgery without removing skin?

Upper eyelid scarring is usually very forgiving, even for people with darker skin. I would actually use non-absorbable sutures that you remove 5-7 days after surgery, as these do not cause a reaction with your skin and tissues.

For transparency, revision surgery is always tougher than primary surgery, and the outcome is more variable.To lower the crease, you need a second lower incision with wide release of the scar of the existing crease. You may need multiple procedures +/- skin graft and/or fat transfer.You have beautiful eyes. Sorry for what you’re going through.
