Statement: I feel so fortunate for the training I received and the refinement many years of practice and teaching has afforded me. I knew that working with the face was something that is a little harder than other areas, but with continued focus, high success can be found.

I feel most people would rather look a little tired or less than perfect, rather than looking silly or overcorrected. I try to find how little it takes to create or restore confidence in one’s appearance. To be given someone’s trust with their appearance shouldn’t be taken lightly.

Personally, seeing the glow of new confidence in a patient is the greatest joy for me.

What other surgeries should I get besides a rhinoplasty?
Facial balance
Mar 3, 2021
By your photos you are young and pretty. To compare your facial shape to models I feel yor lower face is smaller. Consider a simple chin augmentation. The rhinoplasty would certainly involve extensive cartledge grafting to the bridge to ADD to your face to gain harmony.

Botox should be your secret
Mar 3, 2021
When too much Botox or the wrong areas are injected, others will notice and snicker. You were right not to go back. Upselling is TAUGHT as a part of most injection training. Be sure your provider listens and explains the procedure. Most busy competent injectors should try to see how little units are needed and for a first timer...so something predictable. So ..knowledge of anatomy beats out high doses of Botox. The worst thing is...your secret is out. It will soften in weeks and take pics to...
Is Botox an Effective Option for Wrinkle Prevention?
Botox is for patients that already have wrinkles.
Mar 3, 2021
The obvious solitation that "pretox " is a thing is really bad. The results of Botox or Dysport to reduce lines of expression in the upper face are somewhat cummulative if you keep it up. The fact is that if a friend started "preventative Botox " at age 23 and her friend started at 33, the one starting later will catch her friend in a few treatments. The idea of tell you people that have to get started early other than sunscreen and eating well is a disservice. It is spreading anxiety and...
Will laser resurfacing help my sun damaged thin skin at 30?
laser resurfacing
Dec 29, 2020
On this topic to lessen wrinkles and smooth skin tone there is a real truth, there are no short cuts. Treatments with little downtime offer little change, so match your needs to the laser treatment. Fractional CO2 laser resurfacing is the gold standard but it can be adjusted up or down. Erbium laser heals faster as is suited to more subtle changes. Mechanical dermabrasion is still an old trusted tool reserved for scars and difficult lip lines.
Good luck

Is it worth getting my deviated septum repaired?
Septal deviation
Dec 29, 2020
The key here isn't if you would benefit from a slightly difficult septoplasty but how to do this without insurance in an era when the hospital cost normally exceeds $30,000. Most facilities offer self pay rates that are negotiated. Here costs are less than $2000. The surgery must maintain support of the tip of your nose while moving it to midline to open both sides for breathing. This makes it slightly like a rhinoplasty but skilled ENT surgeons are capable as well as others specializing in...
What can be done to help? In need of revision rhinoplasty!
alloplasts / in the tip of the nose
Dec 13, 2020
Artificial materials are always an inferior choice to your own body's cartledge. Sometimes it is worth the shortcut, like in the bridge. In areas that move and have very little separating it from the outside world, infection or extrusion await. It is difficult to comment on your aesthetic outcome as one would need to see where you started.
good luck,
Smythe Rich, MDFacial Plastic Surgeon
I would like to correct the symmetry of my nose, any suggestions?
Crooked nose, not for beginners
Dec 13, 2020
Many surgeons can reduce the hump and narrow the bridge. But standards for everything have gone up. To correct the end of your nose requires advanced techniques like septal replacement grafts and the middle needs spreader grafts ideally available if the first surgeon is aware not to discard the cartledge from the septum. The bones at the top are asymmetric . Your right side is nearly vertical and the left is oblique necessitating reshaping these bones.

Brow asymmetry
Jul 31, 2020
You are correct about this common facial trait. If you are highly motivated, a unilateral endoscopic browlift may help. However, as you may imagine, we surgeons never get things perfectly even.If the brows start quite different it is easier to help. You do also have a little ptosis of the right eyelid. That also can be addressed if this worsens to the point of affecting vision. Personally, I would perform the brow repositioning first and then re-evaluate. Finally, even if you dont kike the...
Brow lift recommended for angry eyebrows? I'm 33 years old, what would be the longterm implications
Endoscopic browlift
Jul 31, 2020
One's brow position conveys over 30 emotions. When lower or arched downward,it may portray an unwanted emotion. Browlifts should be carefully considered in males, however if the brow position is particularly low, an endoscopic brow lift can be welcome change. I advise patients to access the resting brow appearance and then wrap their head in a towel/ turban and compare. Raising your brow height moderately would not be unwise.in your case this isn't age related, but a family trait. Good...
I think I have a weak chin and double chin. Should I do Kybella or chin fillers?
Chin augmentation
Jul 9, 2020
If you speak to a qualified surgeon they will definitely suggest a chin implant. Perfect..permanent..and rare problems. You will spend much more and have less effect with injectables. The perception of fat under your chin is just redundant unsupported tissue that would disappear with a stronger jaw line.

Recessive chin leads to mouth changes
Jul 9, 2020
The common answers of botox for the depressor muscle or filler will be brief and partially correct the concern. A correctly sized chin augmentation will release the ligaments pulling the corners of the mouth down and more importantly produce a better jawline. This is permanent and rarely but easily removed.
Good luck

Can a prominent antihelix fold be fixed after a previous otoplasty?
Revision Otoplasty
Apr 27, 2020
It seems as if a very conservative manuver was done. The curves are soft without bossa but the large choncha persists. Further excision of cartledge with retrodisplacement of the inferior antihelix is needed. Your assessment is correct. Some otoplasties require revisions. It can be addressed.

Softform Vs Ultrasoft Implants for Permanent Lip Augmentation?
Artificial appearance with lip implants
Dec 15, 2019
I quit placing lip implants soon after fillers arrived in 2004. Even happy patients on exam; the implants are palable and often distort upon animation. Many come back for removal because of a "battery cable" presence. The best longer lasting choice is fat transfer.

When does swelling go away after closed nasal reduction fracture? And can I get my old nose back aft
nasal fracture
Nov 20, 2019
Nasal fracture closed reduction is the best attempt to improve the shape and function of the nose by pushing and pulling the bones before they set in place. Since not cuts are made little swelling is seen. Some swelling will be possible but resolve with in a week or 2. The final shape many take months to a year often later revealing imperfection hidden by the fullness. Cartledge components are difficult because they will not snap back into place. Those fractures require time for healing/...

Voluma issues
Nov 13, 2019
All HA fillers have problems but the Vycross line has had a little bit more. All these issues are limited and patients have been fine. Hyaluronidase injection to dissolve the filler are helpful. Please seek someone who has seen this problem before.
