Is there any way to keep the fillers in your lip looking at its best for a longer period of time?

Lip augmentation with Volbella (also Juvederm and Restylane) should last close to 1 year. This is obviously an average length of time and some people tend to be fast metabolizers. Most patients like the initial swelling that occurs during the first week. The slight decrease in size that occurs after about 1 week is normal and is not an indication of the filler not lasting. If you want a greater augmentation, additional filler can be placed at any time.

From the photos, it doesn’t appear that you needed a facelift before and I would not recommend any revision lifting. Depending on your exact concerns, there are other (less invasive) procedures that can help define and contour the face. I recommend you voice the concerns to your prior surgeon but also consider a second opinion from an experienced facial plastic surgeon.

As others have mentioned, the swelling can take several months to resolve and speaking directly with your surgeon about the concerns is recommended. He or she can explain how the areas were addressed and what to expect. Everyone has some degree of facial asymmetry and the nostril height can be very difficult to correct completely.

Revision Facelift

Thank you for the question. I assume that you had a limited “mini” facelift based on the photos and the shorter duration of improvement. In order to achieve the result that you’re looking for in the lower face, I would recommend a deep plane facelift (+/- cervical liposuction) and fractionated CO2 laser. For more information, an in-person consultation with a facial plastic surgeon who has experience in revision facelifting would be beneficial.

With just having had the cast removed, you definitely have a lot of swelling present. I counsel all of my patients that changes can occur in the nose up to 1 year from the surgery. Additionally, I like to follow my patients long-term as the swelling resolves. For your concerns, I recommend researching and scheduling an in-person consultation with an experienced revision rhinoplasty surgeon. Depending on what type of surgery (or surgeries) you’ve had in the past, the treatment can vary slightly. I do agree with you that fillers are not a good option and I do not recommend them in your case.

From the limited photos, it appears that you have asymmetric swelling and not nerve damage. This asymmetry can be fairly common and last several weeks during the healing process. You may also notice that it fluctuates with activity levels, positioning during sleeping, etc. Throughout this early postoperative stage, I recommend that you follow up closely with your surgeon who can help guide you on expectations.

From the limited photo, it seems that the “lines” are the underlying platysma muscle, which is common to see in thin patients. It is also completely normal to have a tight sensation across the neck as the muscle appears to have been bound medially and then pulled laterally. I recommend that you continue to have close follow-up with your surgeon until all of your concerns are addressed and resolved.

It appears that you have some mild facial asymmetry, which is much more common that most think. For this, I would treat directly with filler to the right nasolabial fold. I do not recommend cheek augmentation or buccal fat removal to address this concern.

From the photos, I recommend starting with botox to the masseter muscles. It appears that the fullness is located over the angles of the mandible rather than the medial lower cheek region. This would narrow the lower face giving a more heart-shaped appearance, which is the look I believe you are trying to achieve. I recommend having a consultation with a facial plastic surgeon for an accurate assessment.

After 1 year, you have fully recovered from the facelift. If you are unsatisfied, I recommend you voice your concerns to your original surgeon again. I agree that the neck and midface have room for improvement but a deep plane lift and modification of the deep neck structures are necessary to achieve the result you are looking for. A second opinion from an experienced facelift surgeon may be necessary.

Smoking can negatively affect wound healing and the final outcome. On the other hand, nicotine gum is safe to use. I have never had an issue with gum and any of my facial plastic procedures. Congrats on quitting smoking and good luck with your future surgery.

Under eye filler can help but will not eliminate the wrinkles. Filler is placed over the bone to volumize the infraorbital area. For you, the wrinkles are located in the lower eyelid (superior to the bone). If the wrinkles are only present when you smile (muscle hypertrophy), then botox can help smooth the area.
