There does appear to be some improvement, however if you are hoping for more improvement, and you are not pleased with your interaction with your surgeon, I would recommend seeing a board certified facial plastic surgeon to discuss. You may be able to take more skin on upper eyelid and brow lift if brows are low, which they may be but is difficult to tell on photos.

Hello – It is difficult to determine from just 2 front photos. However, it may what you have suggested, that your eyes are pushed forward and up. This can sometimes be associated with other things, so I would recommend that you be seen by a board certified facial plastic surgeon to evaluate this and they can discuss your options. That may include surgery on the bones around your eye and not your eyelids.

Hello – is the enophthalmos due to previous trauma? There is the option of surgery to put an implant on the floor of the orbit that can correct the defect created by trauma. If not due to trauma, then it is possible to get a CT scan to evaluate the mid facial and orbital skeleton to see the difference. Implants can be built to gain symmetry. Hello – it is actually quite common for people to have asymmetric height of the brow. If you are able to correct this with botox to lift the brow slightly, then that is great and generally all you can do at this point. If you notice the eyelid itself drooping, then that is a different issue called ptosis and sometimes requires a surgery to correct. In time, you may find that you will benefit from a brow lift with asymmetric lift to try and create more symmetry at rest.

Hello – it is very common for people to have asymmetry to the face. With just seeing one photo and not being able to evaluate anything else, it seems like this may be due to the bone structure, which is often the case. However, you would really have to see a board certified facial plastic surgeon to evaluate the eyes, and cheek structures to determine the source of the asymmetry. There are various fillers or facial implants that may be able to help improve the symmetry.

Dark circles under the eyes, particularly in pigmented skin or darker skin tones, can be very difficult to change/treat. If it is darker due to a shadow that rests in a groove between the eyelid and cheek (nasojugal groove or tear trough) a filler in the area or blepharoplasty/fat grafting can help. If it is not due to a shadow, it is likely the pigment of that skin and it is usually not able to be improved very much. One could try a lightening cream like hydroquinone, but that often times…

Hello- HA fillers are being injected into the chin for augmentation. These fillers are injected deeper than those in the nasolabial folds or lips. The chin filler is injected deep by the bone, so yes, I think it natural to wonder if this could be changing the bone structure. There is no evidence of this and it would seem to be very unlikely. Hello- from the pictures you submitted I would agree that changes in facial volume would help. You are still young, so some of the things you note (asymmetry, decrease volume in cheeks) are related to soft tissue, but also skeletal. You could consider facial implants including sub malar and chin. If you want to keep it simpler, you could do fillers, either fat or hyaluronic acid or both. It seems that adding volume to chin as well as cheek region would help.

Hello- as you have correctly noted, these wrinkles are not crows feet and Botox won’t really help. As you also correctly note, the lines are worsened with facial movements associated with smiling, so Botox would have to stop your smile and you would never want to do that! As I view the photos it looks like your right side has more volume than left, If you like that side, I would try to match it with more volume. A filler in the temple will likely not achieve what it seems you are looking for. I understand your thought on this, like Volume replaces volume in cheek and gives a subtle lift as well, would volume in temple help lift the brow. You are young and have likely not lost a lot of volume in your temple yet, so volume does not need to be “replaced” there. You are definitely correct, it appears that you have lost a lot of volume in your mid face. I do think you could undergo a facelift, however it is also important to add volume. I would plan to do fat grafting to the mid face, nasolabial folds, marionettes lines and maybe even the chin a little.
