The result you are experiencing can happen with certain starting breast shapes. To improve the appearance, you will need a revision surgery which may include different implants, a lift, and potentially fat grafting. You can always reconsult with your surgeon to see what he or she says or seek out a few other opinions.

It looks to me like either the implants are sitting a little higher than idea, or the breasts are waterfalling off the implants, or perhaps a combination of both. Sometimes this can be corrected by either lowering the implants, or doing a more formal lift to get the relationship of the implant to breast tissue in a more idea position.

After explant and capsulectomy, the outcome can be quite unpredictable depending on how the muscle attaches, how much the skin retracts, and where the original breast fold falls. Sometimes, a lift with fat transfer can improve the results. I recommend reconsulting with your surgeon to see if either of those is an option.

You can safely have a breast augmentation with the history of a PE during pregnancy. I advise coordinating the surgery with your hematologist to see if any blood thinning medication is indicated for your breast augmentation. Sometimes it is recommended that you hold your oral contraceptive pills if you take them. Many times for patients with your history, no further treatments is needed for a short, straightforward surgery like a breast augmentation.

Yes, you can have an extended tummy tuck, breast lift, and pubic lift at the same time. I think it is important to address the pubic area at the time of the tummy tuck, especially in weight loss patients, so that it does not look bigger after the surgery. Sometimes, a body lift is a better option than an extended tummy tuck to help lift some of the upper lateral thighs. You can definitely have a breast augmentation with a history of MRSA colonization. I use a chlorhexidine scrub in the OR to prep the skin, and use Clindamycin for the preoperative IV antibiotic and oral dose after surgery. No matter the history, there is always a very small risk on implant infection with a breast augmentation, but these precautions can reduce the risk.

Implants placed under the muscle can sit high for weeks after surgery until they ultimately settle into the pocket. Implant massage and a strap can definitely help them settle.
I have patients wait 6 months prior to evaluation for a revision to reposition implants.

Based on the photo, it appears you have a significant asymmetry in the size of your breasts. Depending on whether the right one has gotten smaller or the left bigger it could mean that the right has deflated or the left has extra volume from a seroma. Capsular contractures can form “early” in the post operative period, but this is usually 3 months or so after surgery before they become appearant. A recent study showed that around 40% of capsular contractures happen within the first 2 years of surgery, and 80% in the first 5 years.
