You chin is slightly receding based on this photo. A slight augmentation would be reasonable, but be careful, as too much augmentation will lead to a more masculine appearance. It looks like you had an open approach/external rhinoplasty. Additionally, this is a revision. In my experience, both of these are subject to longer periods of tip swelling. In general, a reasonable idea of the final result can be assessed when the cast is removed of the bridge only. Swelling tends to last longer and be more noticeable in the tip than in the bridge. Nasal swelling, while is can dramatically improve in the first 2 weeks after surgery, can take several weeks to months to fully subside. In general, selling lasts longer in the lower portions of the nose. Discuss your concerns with your surgeon; based on the techniques used, what you see may or may not be expected at this time.

Septoplasty is usually performed to improve breathing, and generally works on the portions of the septum not visible externally. Rhinoplasty reshapes the nose, and often involves treatment of the upper part of the septum, separate from surgery to improve nasal breathing. These two are often combined and not mutually exclusive, but generally have different purposes.

It sounds like you sustained a significant blow. A complete physical examination would be helpful to determine what the best course of action will be. If there is any compromise to your breathing or if you continue to see an external deformity of the nose you want corrected, see a facial plastic surgeon for evaluation. Depending on your symptoms and his/her findings, surgery may be an option.
Will Swelling Be Inevitable After Restylane Injection Under the Eyes?

Treatment with Restylane involved placement of material through a needle or cannula under the eyes. Ideally, this is placed below the muscle and against the bone. Even at this level, the covering (skin, muscle) is still very thin, and the area around the eyes “hides” the normal amount of swelling and bruising less effectively. Generally, these subside in 1-2 days, but occasionally can last 2 weeks or longer.
Can steroid shot or rasping help remove an AlloDerm implant?

AlloDerm does undergo partial resorption over time. For major issues, it is unlikely it will resolve on its own and surgery is indicated. However, if surgery was relatively recent and the problem minor, it may continue to resorb. I would wait at least 6-12 months in such a case before reoperating.

There is always swelling of the nose, I would expect this to be fairly significant 1 day after the cast was removed. Keep your scheduled post-op visits with your surgeon and discuss it with him/her if you are still concerned.
I’m 3 weeks post-op and know to be patient, but are these my final results?

At three weeks after rhinoplasty, there is still considerable swelling. I generally tell patients that they will have a reasonable sense of the final result in 6-8 weeks, a pretty good idea at 3-4 months and a very good sense by 8-12 months. Every patient is different and the degree of swelling is affected by what was done during surgery, but there is still swelling at 3 weeks. It is always a good idea to discuss your concerns with your surgeon.

Even in the best hands, any surgery can have unwanted or unexpected effects. In particular, revision rhinoplasty by its nature is trying to correct a prior problem, and is done in the setting of prior surgery (with prior scar tissue, surgical changes, etc.) However, despite these caveats, in trained hands revision rhinoplasty is generally quite successful.

While it is possible the redness and swelling could be related to the cast and tape and confined to the skin, increasing redness and swelling may indicate an infection. See your surgeon immediately for a complete evaluation. Based on what he/she finds, antibiotics and/or drainage may be necessary.

At your age, a temporary answer means many, many treatments over a lifetime. You certainly could treat this with fillers, as long as you understand the effect is not permanent and will likely need repeated treatments at least every 1-2 years while you are young, probably more frequently as you get older. Swelling at the tip of the nose can take time to resolve, but at 3 days it is not uncommon at all. Significant improvement in the swelling occurs within the first few weeks, and patients are generally quite presentable by 14 days after surgery. However, while most swelling may resolve within the first several weeks, tip shape continues to evolve over the first several months.

As this is your third rhinoplasty, you know that swelling takes time to resolve. Post-operative swelling generally lasts longer with each subsequent surgery. Every nose is different, but based on your prior experience after surgery, you can safely assume the swelling will take an additional 20% longer than the last surgery. Swelling at the tip will make the tip seem more rounded, prominent and often more upturned. Discuss your concerns with your surgeon.
