Hi there, It seems as though the dimpling gets much worse with flexing…….it would be worth a try with a diluted Botulism toxin series of injections to relax those pulling muscle connections…..it is possible to add fat, try to release those tethers but honestly you would spend a small fortune, go through a agreat deal and still not have great arms…….a direct excision with a long scar would also tighten significantly but the scar is a big price to pay……

It appears that the nasal bones are still too wide for the rest of your nose…..your surgeon probably could probably fix this by “breaking the bones” and moving them in line with the lower 2/3 of your nose. This could be done with a light sedation and take only 20 minutes. It would require a splint and might well bruise some.

Definitely see a few Board Certified Plastic Surgeons for consultation and prices…and take notes……they should include specifics of the surgery to be done, Surgeon fees, Operating Room/Facility fees, anesthesiologist fees. I do not believe any reputable Surgeon can properly price your costs without seeing you in person and examining the specific problems you have while developing a good plan of action. Hope you can have it done!!!

It sure looks like the tip cartilage is asymmetric….this could be softened with periodic injections….but in reality the cartilage would have to be surgically modified to even them out….I call this a “Tip Rhinoplasty”….recovery is easy like a few days out of work with little bruising and no broken bones. Of course you need to see a surgeon to make sure that plan is correct and takes all the issues into account

I would recommend you visit a local Plastic Surgeon certified by the American Board of Plastic Surgery. You can have local, IV sedation or General…I prefer intubated general because it is the safest as your airway is controlled by a tube with a balloon on the end of it so no blood can get in your lungs during the procedure. Your problem and question is a common one……when to do lipo versus an arm lift. Hi there….seems like you have the classic issue augment alone or augment and lift. To me you would get a great result with an augmentation alone. Of course the nipple areola would be a little low but they would still point outward and not down. If you want more then you will add significant scars to move the nipple areola up an inch or so. ultimately its your choice but the extra scarring and cost doesn’t seem worth it to me. good luck!
