Recommendations on post-surgical issue from bacteria?

Hard to tell whether you have residual dormant infection or just scarring… bacteria can remain colonized on the cartilage because of its poor blood supply and antibiotic penetration… this makes matters tricky…

I would likely try to fix this in 2 stages… the first a wide debridement and cultures to both make an accurate diagnosis and help clear any residual infection…. depending upon the results of that, I would treat you appropriately before definitive grafting, etc.

You had a masculine and fairly attractive nose to begin with…. sadly, you have been over-resected, shortened and lost all tip projection… a lot of this can be restored/corrected with a revision rhinoplasty and structural grafting with either autologous or cadaveric costochondral cartilage grafting…

Your dorsum and the body of your nose looks ok, it appears to be predominantly a tip rotation and projection issue… this can best be solved by an open revision and likely septal extension grafting

You do not appear to be over-resected, and you are certainly shortened and over-projected… tip adjustments for projection are generally achieved bu ‘tongue in groove’ techniques over the caudal septum or septal extension grafting … sometimes adjustments in the tip cartilage are made in addition to fixing them in a new location

Your issues can be addressed by lowering your radix and providing tip projection to fix your ‘polybeak’… a septal extension graft will take care of this….

Your plunging tip is the consequence of poor support… the likely solution to this issue is a septal extension graft which (at least in my hands) is most accurately placed by way of an open approach..

The ‘bowing’ of your columella and hence the medial crus of your lower lateral cartilages is likely secondary to either a short/deficient caudal septum or one that is displaced…. In order to fix this, you will need structural grafting likely in the form of a septal extension graft to address this

The twisting and collapse are secondary to loss of support… you would likely benefit from a revision with structural cartilage grafting to support your dorsal and tip to restore symmetry and dorsal support

You have injured both the columellar skin and the underlying medial crus of your lower lateral cartilages in the setting of inherently poor tip support

In order to fix your deformity, it seems you would likely need structural support for the caudal structures which would likely take the form of septal extension grafting or columellar strut grafting

At some point in your rhinoplasty journey your nasal tip cartilages were trimmed… when this resection is aggressive or combined with the removal of upper lateral cartilage (particularly at the scroll area) alar retraction is the result…. This issue is compounded by your tip crookedness which is likely the result of poor support

In a revision rhinoplasty, scar pulling up your alar rims can be removed and a spacer graft or lateral crural strut graft can be placed so that nature does not fil…

Both your tip and dorsum have been over-resected… the loss of support of your septum is resulted in the ‘waviness’… your aggressively resected tip cartilages are what makes it crooked and unnatural looking

These issues can be addressed with a revision rhinoplasty that must include structural grafting to support your weakened nose

Your keystone area where the septum meets the nasal bones is likely disrupted and your cartilaginous septum is warped…. the best way to remediate this is with robust structural grafting (spreader/splinting grafts preferably from rib cartilage)

Blue eyes and freckles …. Classically thin skinned and not a great candidate for steroids

You skin should recover a bit, and a revision by an expert is your best option… you will require dorsal support with spreader grafting (cadaver or autologous rib) plus a ‘fascia blanket’ to add some soft tissue to your dorsum

Those look a lot like onlay tip grafts especially visible with soft tissue atrophy around them …the only reasonable way to approach this is by a revision rhinoplasty whereby an expert can take things apart and start over to remove those visible grafts and give you the structure you need…
