Perform an internal lift without any external scars
Your tissues should retract and shrink
Wait six months to decide what type lift is best
I have seen amazing retraction after removal of submuscular implants without lifting

Do yourself a favor
Avoid the tuba procedure for the many reasons stated by the others.
Having performed over 1000 revision surgeries I can testify to the fact that far too Many breast augmentation revisions stem from this approach .
Would need to see you to judge size That best fits your body based on torso length etc

Many factors can cause anxiety after breast augmentation
Especially with the ridiculous nonscientific literature that is present throughout the Internet .

Be patient As I expect you will be fine because you did not go too large in size .
I would give it at least 8 months before making any decisions regarding explantation.
Your body will bond with the implants within two months and they will no longer feel foreign to you .

Totally disagree with you going bigger .
You will look heavier and less athletic in my opinion .

But give it at least 8 months you may be surprised how much you like the size over time .
Also you must increase at least 120 cc or change the profile to a high profile
Implant if you decide to upsize .

Except slight greater risk of asymmetry long term after replaced .
If you need to wait I would have your surgeon easily deflate the other side in the office without anesthesia until you are ready .
This way you avoid risk of asymmetry

Deflated implants have no real risk left in the body until best time for your surgery .

You should be more Concerned about the shape over the size and scars .
Your breast now only make you look heavier as they are too big and low
You will definitely need a lift with downsizing .
Gummy Bear is NOT the answer and will make them worse .
A well done circumvertical lift with minimal scarring and breast lift with or without ADM grafting is your best option for long term results .

Remember that a saline implant is roughly 10 percent larger than the same size silicone implant .
So a 390 saline is equivalent to a
420 cc silicone assuming similar base diameters .

Would need to see you breast to determine how much sag you will have with smaller implant volume

Of your breast tissue and autoaugment your breast without an implant .

I do believe you have a variant of a type 1 tubular breast deformity but regardless the treatment for you would be the same .

You are a very straight forward case
Easily Remedied by a circumvertical breast lift .
expect a great result .

Surgery is your best option. You would need an open rhinoplasty with tip soft tissue debulking and tip grafts. You also need removal of the wide excess nostril skin. You could achieve a non-invasive but much cheaper effect with appropriate make-up. You would need a make-up artist to show you how to make a wide nose narrower. This of course would be an illusion but a quick fix without spending much money. The cost of this could range from $4000 up to $15,000. I would say that…

I feel that performing a left at this point is easily achievable. You would need smaller implants placed in a neo-sub pectoral pocket after removal of your large implants. I would decrease the size of your breast at least 250 mL and perform a concomitant vertical short scar breast lift .
