Boy, if the graft edges are sharp enough to show up well and be a problem, putting stuff around the edges won't keep the edges from still pressing up against the skin. The added material will go *around* the edges and the edges themselves will still be a bother, and now your tip will be much larger, too, probably not something you're looking for. You have a milder version of the wide tip cartilages in the before and after photo I posted with this answer. The two cartilages that form the tip of your nose are broad and strong, and they make the tip look wide-ish. When the tip cartilages are like that, you can often also see the little dent between the cartilages where they meet in the middle. If you like the new skin care, don't stop using it.

Besides the wide tip cartilages and the wide placement of the upper lateral cartilages, it looks like the upper third of your nose, the nasal bones, are also widely-spaced. But narrowing the bones is usually rather predictable in a rhinoplasty, so that's good. Of course I haven't had the chance to feel your skin, but your photos show a nose that looks like average less-thick-than-a-teenager's skin, and lots of adults have this operation. You also seem to be a very good candidate because you have features that are, in expert hands, usually rather predictable to improve: the bump on the bridge, but also the length of the nose and the prominence of the tip.

What I see is that the tip and lower half of the nose still looks wide-ish. If that is no longer swelling, but rather the size and position of the cartilages, then those cartilages can be modified in a revision. But I don't see a profile view to evaluate the length of the nose, and it's typically the 3/4 views that show the width of the tip best.

Remember that rhinoplasty is an exquisitely difficult operation to get right, and you should only have surgery if you are able to make yourself...

I know, I don't have a complete set of photos from all angles here, but it seems to me that the two cartilages that form the tip of your nose are strong, and sitting in a wide position, and it's those cartilages that hold the alae where they are, making the whole bottom third of your nose look wider than you'd like to see it. From these pics, it seems to me that you might consider a rhinoplasty; I don't see that an alarplasty would change anything for the better.

I see three main areas to address on your nose. You have that bump on the bridge. Your nose is long, with the tip hanging down more than perhaps it needs to. And the profile view shows, in addition to the low tip, that the tip projects forward away from your face strongly. Bringing the tip back closer to your face helps in making the nose look less prominent.

The nose naturally tends to get longer with age, as gravity pulls on it over the decades, so it's not unusual that you would think it's longer than it was. Was it this over-projecting *before* the 2nd operation? If the surgeon only put in a "tiny bit" of cartilage, you wouldn't think it would make a huge difference in the projection. I'd love to see some pre-2nd-surgery photos. The contours of the tip of your nose seem to indicate that you've already lost the great majority of your swelling, so I'm not so sure that any more deprojection will happen with more time. Could there have been a big graft that was placed all along the base of the nose?...

Filler would try to mask the bump by filing in the relative depression just above the bump. But when you put something in, it makes things look bigger. So yeah, it might be harder to see the bump, but your nose would look substantially larger, and I doubt that's what you're looking for. Do a Google search for "Roman statue facial profile" to see what happens when you fill out above the bump.

In a regular rhinoplasty, the bump would be lowered, and the tip elevated.

You already know that your nose is too projecting, especially in relation to your bridge. You might be underestimating how much tip elevation your nose can use, however. When a nose is deprojected, it has a tendency to make the nose *look* longer, as though the tip had actually dropped, so a goal would be to get the tip up enough that it looks good with the newly-deprojected tip. When a nose comes out strongly forward away from the face, we call that a "projecting" nose, and we want to "de-project" it, by bringing it closer to the face. A "long" nose is a nose that is long if you measure from where the nose begins between the eyes, down along the bridge to the tip. So a long nose has a droopy tip.
