Is the profile view shown in the example "after photo" possible, based on my current profile?
Forward sloping nose-lip junction
Sep 10, 2020
The pictures you show depict a nose-lip junction that slopes forward as a result of plentiful caudal (inferior) septum and/or nasal spine. Some patients do not mind it while others do. Rhinoplasty can indeed significantly reduce this forward sloping and produce a more vertical upper lip-nose junction. The deprojection can be limited by maxillary/upper dentition anatomy. Careful consideration of course must be made to ensure that other areas of the nose (bridge, midvault, tip) are...
Can sliding genioplasty be done through submental incision rather than intraorally?
Intraoral versus submental incision for genioplasty
Jul 18, 2020
Sliding genioplasty (either for advancement or for reduction of the chin) can be performed via both intraoral or extraoral (submental) approaches. The choice of approach depends on the desired changes to the chin, the physical characteristics of the patient, and the personal preferences of the patient and surgeon. This decision would be made after a thorough consultation. The submental incision for sliding genioplasty is slightly longer than what might be employed with a silastic chin...
Microneedling RF effective for me to tighten neck and chin, or is surgery best?
Neck muscle banding best treated with office surgical necklift
Jan 31, 2020
A formal in-office evaluation and consultation are necessary to provide medical advice, but based on your photos alone, it appears that you have neck platysmal banding, which is best treated with a neckllift corset procedure, typically performed under local anesthesia in office in my practice.  We typically see that radiofrequency tightening or microneedling cannot tighten the muscle bands and can actually cause fibrosis under the skin that can make a necklift surgery more difficult in the...
Cheek fillers?
Cheek augmentation with fillers
Jun 14, 2015
Fillers indeed can reliably provide the appearance of higher cheeks, which can provide a facial appearance that is considered sexier (in the younger patient) and more rejuvenated and less tired (in the older patient). Several fillers on the market are available depending on considerations on the costs and the desired amount and longevity of the augmentation. A comprehensive evaluation in-office is required.
I think my nose is too wide, what needs to be done to my nose for a better looking appearance? (photos
Rhinoplasty for wide tip and nostrils
Apr 27, 2014
Your pictures do not show your whole face and hence it is tough to comment on how your nose lies in relation to you face. Patients might prefer a narrower tip and nostril compared to yours currently, but other patients are fine with the way they look. Ultimately, the decision to undergo changes to your nose and face is a very personal one that you must contemplate and prepare yourself. The best way to know what type of aesthetics you desire for yourself is to undergo a...
Do older women (60+) who have facelifts still need dermal filler injections after the surgery?
Fillers after facelifting
Apr 16, 2014
Surgical face and necklifting would help to shape the face and neck on a larger scale, while nonsurgical or minimally invasive procedures such as Botulinum injections, laser or chemical peels, fillers, and medical-grade skin care products would be used synergistically with surgery to reduce the appearance of wrinkles, age/sun spots, and volume loss. These minimally invasive treatments can be undertaken either before or after surgery. If performed before surgery, these treatments can...

Nasal hump reduction
Apr 16, 2014
Instead of asking what other people might like for your nose, the best way for you to visualize what YOU might like is to undergo a comprehensive evaluation followed by a goal setting session in front of a computer simulator, where you can pick goals that match your aesthetic taste and discuss the feasbilities of reaching those goals. You might find that a chin augmentation can be helpful with the hump reduction to balance your face. Be careful about reduction of your nose without...
After botox I still noticed my eleven lines between my eyes - could I get a filler then botox?
Eleven (11) lines after Botox
Apr 5, 2014
If you have very deep 11 lines in your glabella, then it can take a few cycles (12 to 18 months) of Botox treatment for the 11 lines to soften or clear. I usually recommend to my patients to be patient as their glabellar muscles weaken with the continued Botox treatment. For more impatient patients, resolution of the 11 lines can be hastened with a number of skin resurfacing or skin care product options in our practice. I do not typically recommend filler to the gabella...
Is there a chance of losing one's sight with upper eyelid surgery.
Risk of vision loss with upper blepharoplasty
Jan 26, 2014
The risk of vision loss from upper eyelid surgery is extremely rare. The risk of vision loss related to bleeding into or behind the eye is essentially zero if your surgeon simply removes eyelid skin or a little eyelid muscle. The risk is slightly increased if the surgeon removes fat from behind your eyelid muscles or if during aggressive muscle excision, the fat compartment is violated and bleeding occurs in the fat compartment without it being controlled. Aside from...
Septoplasty costing $47,000. Is this normal?
Costs of medical care
Jan 20, 2014
If the costs of $47000 were billed to insurance, understand that insurance only reimburses a practice and the facility about one-third of that charge. Meanwhile, out-of-pocket costs (like in self-pay situations) are typically much less than what you are quoting.
What's the best treatment for slight jowls and slightly receding chin? Could a very mild chin augmenta
Treatment of the slightly recessed chin and mild jowls with filler vs chin implant/facelift
Dec 28, 2013
If your recessed chin and jowls are mild, your may want to consider filler placement to the jaw line area. Artistic placement by an experienced practitioner typically can yield a very natural appearance that many patients would not want to trade for chin implant and face lifting. Certainly, the options for chin implant placement and face lifting are there should you not wish to undergo more filler placement, but if you desire a minimally-invasive, little-downtime, inexpensive...

Rhinoplast for the large droopy nose
Dec 28, 2013
Rhinoplasty's most common indication is in treatment of the droopy nose with large hump. Treatment can typically be achieved in one surgery. Avoidance of the inverted V deformity is achieved by your surgeon being experienced enough to diagnose the tendency for an inverted V to occur in your case and in performing compensatory grafting if your tendency to develop an inverted V after hump reduction is high. A comprehensive evaluation with a discussion of your aesthetic goals...
Judging from my face, would I be a good candidate for rhinoplasty? If not, what should I do?
Nasal and chin surgery
Nov 16, 2013
It is not uncommon for individuals to have both nose and chin surgery together if they feel their face needs more balance. Looking at your photos, you have features that have caused many patients of mine to request rhinoplasty to deproject their nose and a chin implant to augment their chin and jaw line. It is best that you have a comprehensive in-person examination so that your concerns and goals can be properly elucidated. Also, you would have the opportunity in our office to view...
Rhinoplasty + Chin Augmentation (Pictures). Can my Chin Be Worked On? What Does my Nose Need?
Rhinoplasty with chin augmentation
Sep 29, 2013
Rhinoplasty and chin augmentation are a relatively common combination of surgeries because of the relatively common facial variation where a larger nose exists with a smaller chin. Many people are fine with this appearance because it makes them distinct from others, while other people might wish to change their appearance. The amount by which you wish to reduce your nasal size and augment your chin is highly individualized and is in fact customizable to a large degree. The best way to...
Will This Pulled Look Relax and Correct Itself?
Unwanted pulled look after facelift
Sep 18, 2013
Your pictures are somewhat limited and have some central bulge lens artifact. Swelling after a facelift can take weeks to months to settle down depending on the patient. Therefore, judging your appearance 2.5 weeks after surgery is usually premature. However, on your right side, at this stage postoperatively we typically would like to see a more even appearance to the face and neck after a facelift, such that the lifting vectors are distributed...
