The dynamics of your smile show the projection may be due to soft tissue( muscle and fat) over the chin bone. Therefore you will need a through evaluation with 3-D CT scans and properly planned procedure to balance your nose and chin. Too much bone removal can cause disastrous result later in your life .

You have to understand that nose is a 3-Dimensional structure. The structure that determines the straightness is the nasal septum which is made of bone and cartilage. The cartilage has memory and spring back tendency. If one has a preexisting septal deviation and if that condition is not addressed during rhinoplasty the nose will eventual veer to the side of the deviation. It is not “environmental” due to global warming etc. Incomplete evaluation and treatment is the cause.

Your pictures show an unusual wideness of the dorsum(bridge) for an open rhinoplasty procedure. May be your osteotomies were incomplete or not done at all. “Open roof deformity ” is caused by incomplete osteotomies. The nose is a pyramidal structure formed by the two nasal bones fusing in the mid line. Did you have a non incision double lid operation or incision procedure. Any way the the skin is fixed at a higher level than the ideal location causing the problem you have. So you have two options 1 wait it out for the scar to soften up or fade away or2. Have a revision surgery BTW where was the surgery done. You should also revisit the doctor and get his or her advice also.

There is a congenital condition called Hemifacial Microsomia that can distort the face as you grow up. It can manifest in various degrees , from gross deformity to some degree of facial asymmetry. If it is Hyaluronic acid (HA) filler it may be dissolved by injection of Hyaluronidase(Vidase), an enzyme that dissolves HA incrementally till you get the result you want. If it is Radiesse or MMA based fillers you may have to wait it out or undergo complicated surgical procedures to remove it. Results may be worse than where you started from. Good Luck.

If you had a negative sinus scan the pain is not likely from your sinus. First a good front and profile views of your chin with and with out smiling would help in analyzing your problem. As you are a young person it is not an aging change.From your description it appears to be caused by some malfunction of mentalis muscle. So it can be fixed by restructuring the muscle , fat pad etc.. Hope that answers your question.

Considering your age and your facial configuration a genioplasty with sliding osteotomy will be the best option. It is a permanent procedure rearranging your own chin bone. Everything else will be problematic sooner or later.
