At 3 months postop you have significant lower lid retraction and also some apparent chemosis (the bubble in the lower outer corner).  You need some support to your lower eyelids.  Early on you may be able to do some massage to relax the scar tissue, but you may end up needing a canthopexy or lid tightening procedure for your lower eyelids.  You should definitely follow up with your surgeon soon.

For neck aging changes nonoperative treatments are not very effective.  You would do well to have an open neck lift performed with repair of the platysma muscle to improve the banding.

If you do require both procedures I would recommend doing the rhinoplasty first.  Some of the maneuvers that may lift up the tip may slightly help your upper lip.  If not, a lip lift can then be performed.

At this point it is unlikely that your rib graft will warp.  My experience with these grafts is that they will slowly sort of melt away with time and you may require a revision procedure in the future.

It is almost certain that your nasal implant is infected.  If left unchecked it will almost certainly extrude through the tip of your nose.  I would recommend that you have it removed and then you can revise it with your own tissue.

You are obviously very early after your procedure and should continue to follow up with your surgeon.  Most likely the bump that you are seeing is a small bone spur that has been left behind following the surgery that is now showing up as the swelling is resolving.  Your surgeon should be able to make the determination whether something will need to be done to correct the issue or whether it will resolve on its own.

It is not clear whether the columella strut placed at the time of your primary rhinoplasty or an inadequate shortening of the caudal septum is responsible for the appearance of a prominent columella and short upper lip.  This really will need to be determined at a consultation.  You would seep to benefit from some further narrowing of your nasal bones and support of the middle vault with some spreader grafts.

My suspicion is that your surgeon put a graft of cartilage in the area at the base of your septum and this has caused some swelling versus muscle weakness.  I would guess that the problem would resolve completely.  Please follow up with your surgeon.

To answer your last question first, I don’t think you would be a good candidate for a nonsurgical rhinoplasty.  The procedure would be pretty straightforward with a reduction of the nasal bones and narrowing of the nasal bones, which would help with the bone/cartilage spurs that you do not like.  You may opt for some refinement of the nasal tip at the same time.

I would agree with your surgeon that in thick and oily skin the tip of the nose may remain swollen for a longer period of time.  Please follow up with your surgeon to see if some steroid injections may be of value.

At six weeks postoperative it is very unlikely that your ear shape will change.  With only a frontal view it is difficult to completely diagnose the problem, but it looks as if the middle and lower portions of the ear have been folded back too tightly or too much cartilage from the bowl if the ear has been removed.  It is possible that the ear may be revised but without other photographs it is difficult to exactly state what would need to be done.

I agree that you will likely require the use of cartilage graft to support the cartilages of the tip of your nose.  If the septum was not used at the time of your initial procedure then it may be available.  Otherwise the use of some ear cartilage or rib graft may be required.

Your photographs show a very straight and unnatural appearing anti helical fold. It would be possible to revise this with a revision procedure and replacement of sutures to create a more natural curve. After that was performed additional sutures or cartilage removal could address any prominence.
