As long as you are healthy and have realistic expectations your risks for a septorhinoplasty are not typically increased at age 47. You would review risks, benefits and expectations at a consultation with an experienced rhinoplasty surgeon who would also address the septum.

Your tip is pulling down in the photo of you smiling. Many times that is secondary to a lack of tip support. Go see your surgeon. Most surgeons, including myself offer no charge surgeon fee revisions of their own work , though you may be responsible for the anesthesia and room fee paid to the facility.

I recommend patients wear a wrap, like an ace wrap, to support the neck continuously for one week, and as much as they can stand it for the second week. Yet, I also tend to lift all the way across the neck to try to improve as much neck and under the chin laxity as I can at the time of surgery. The wrap supports the neck tissue in place, and helps to diminish swelling.
Advice needed on septoplasty. Deviation in part of septum which ENT claims can't be straightened?

The septum and the nose are both deviated in this picture and drawing. Repair would necessitate a setorhioplasty including osteotomies, possibly a spreader graft, and possibly caudal end transplantation. I would suggest consulting an otolaryngologist who also does rhinoplast yto treat this disorder.

In these cases structural grafting, like tip grafts, skin thinning at the time of surgery and serial low dose steroid injections in the tip can improve the outcome. See your doctor. By your description it may just be swelling which should resolve over the year. Sometimes a small amount of a low dose steroid can be injected to speed up the process. Your surgeon would be the best to evaluate this option.

Your nose is still in the healing phase so I would not worry at this point. You should see your doctor on a regular basis as the healing process progresses and check with them about concerns at each visit. Things do change quite a bit in the first year of healing.

Open or closed approach depends on the surgeon, and the problem causing the deiviation of the nose. If only the bones are deviated a closed approach with osteotomies may suffice. In your case both the septum and cartilages of your lower nose are bent. I would approach this type of case through an open approach with spreader graft or grafts, septoplasty, osteotomies, and any other techniques needed depending on the appearance in multiple views. An examination would be required to determine how much of the area is comprised of fat versus muscles from the floor of the mouth. Liposuction works the best in people your age because the skin has great rebound and turgor. Go for an examination to learn if you are a candidate.

You are an excellent candidate for rhinoplasty to remove the bump, refine the tip and improve symmetry to the nose. I suggest patients plan a rhinoplasty at least 4 months before a major event like a wedding, as the bulk of the swelling is resolved by then. Understand that full swelling does take a year to resolve, but from 4 -12 months after the changes are subtle.
