How Long Before the Lower Incisions Heals After Transcutaneous Bleph Procedure?
Healing of subciliary or transcutaneous lower eyelid incisions
May 27, 2011
As a facial plastic surgeon, the physical examination determines whether a transcutaneous vs. tranconjuctival approach is used. I have performed both and understanding the anatomy of the lower eyelid is paramount in a successful transcutaneous surgery. In my experience, the incision heals very well with redness being the longer lasting issue. Usually, I find my incisions look great within 4 weeks. Makeup can be applied and camouflage any lingering redness. It is usually the swelling of the…
Columellar Strut – When is It Necessary?
When should a columellar strut graft be used?
Apr 25, 2011
As a facial plastic surgeon, I perform my septorhinoplasty by the open approach. I usually harvest a strut graft from the septoplasty portion of the procedure. I prefer using the patient's cartilage rather than an alloplastic material. Depending on the approach (open vs endonasal), once postoperative swelling resolves, your tip might drop slightly. Other maneuvers such as dome suturing can narrow and project the tip and if these were not performed, your tip may appear like…
Hanging Columella
Fixing the hanging columella
Apr 25, 2011
The hanging columella is easily addressed after a thorough examination. It is important that I examine each patient since the cause of the hanging columella maybe due to soft tissue or cartilage. During the examination, I can diagram the pertinent antomical issues.  If it is secondary to cartilage, I determine whether it is septal cartilage or the external lower lateral cartilage. Removal of excess tissue and suture techniques will work for  soft tissue issues but cartilage…
How Long Does Skin Reshaping Last After Rhinoplasty?
Revision Rhinoplasty requires longer tme for skin redraping
Feb 11, 2011
As you can see, there are varied answers depending on the surgeon. Time will depend on whether an open or endonasal approach was performed. I perform only open approach, therefore, I tell my patients that skin re-contouring may take 6-12 months. After 8 weeks, I have my patients perform gentle massage which aids in lymphatic drainage. Regarding this particular case, there is an excellent chance of the skin re-draping over the next 6 months. In light of the thicker skin and a revision…
Surgery to Shorten an Overprojected Tip – Risks? Technique? Results?
In Rhinoplasty, I look at the entire nose.
Oct 28, 2010
During my rhinoplasty consultations, I take into consideration the patient's desires, their height, their face (round/thin), and chin projection. All of these areas will dictate how the nose will appear after surgery. If the tip is over projected, the goal is to bring it closer to the face but it is important to understand the relationship with the chin. Equally important is to create a smooth and natural transition from the bridge or dorsum of the nose to the tip, nasociliary line….
Botox for under eye wrinkles?
Botox works for fine lines of the lower eyelid but with caution
May 28, 2010
I will use Botox in small units to decrease the fine lines of the lower eyelid. Care should be taken since too much Botox may result in decreasing eyelid tone producing a droopy eyelid. There are other techniques to reduce the wrinkles. In my practice, I will incorporate Thermage and Fraxel. When non-invasive procedures fail, recommend surgery to my patients. The surgery is a lower lid blepharoplasty.
Generally, how long will Fraxel laser results last?
Fraxel treatment results longevity
Jan 11, 2010
I have been using the Fraxel RE:restore for 5 years and have treated pigmentation of the face, neck, declotte, hands/arms, and shoulders with great success. If the patients follow our instructions regarding suntanning and use of UVA/UVB sunblock with an SPF of 30+, their results can last years. This is also very dependent on the area of the US you live. My practice is in Tampa, Forida and the sun is very strong and watersports prevail. Therefore, my patients understand the need to re-apply…
Botox for forehead lines on a 21 year old?
Botox at 21 is safe
Jan 11, 2010
Botox is safe at 21 years of age. I think several important issues to consider are degree of wrinkle formation a patient, sun exposure, and genetics. Deep wrinkles of the forehead and glabella can occur as early as 21 years of age and combine that with the amount of sun exposure and family history, makes that patient a good candidate. Application of Botox should be performed by a physician well trained in facial musculature and who understands the risks associated with Botox and how to…
Keloid removal on earlobes?
Keloid removal of the earlobe
Jan 11, 2010
I have removed many keloids from the earlobes of both men and women. The removal of the keloid is not difficult but the post surgical management is crucial. I often use steroid injections during and weeks after the actual procedure. On occasion, compression of the area is needed. After successful removal, I do not recommend piercing the earlobe in the future. This can only lead to recurrent keloid formation.
How effective is Thermage on the neck area?
Thermage works great on the neck
Jan 11, 2010
Thermage is a non-invasive, radiofrequency technique to tighten skin. I have incooporated Thermage to tighten face, neck, upper arms, buttocks, inner thighs, knees, and hands with great results. There are two generation Thermage units and I recommend checking with your surgeon/physician as to which unit he/she has. Having the second generation unit makes the procedures faster and better tolerated by the clients. Thermage has reduced the need for submental liposuction in many patients. Until…
Are there risks in flying after Rhinoplasty?
Flying after Rhinoplasty
Jan 11, 2010
The issue is pressure changes when flying, in particular, upon descent. I usually require my patients to be available for two weeks after any cosmetic surgery. Regarding rhinoplasty, the two issues are pressure discomfort and potential nose bleeding. Your ability to ‘pop your ears’ can be affected by rhinoplasty. This can lead to ear and facial discomfort during flight. Bleeding from you nose is a risk but after two weeks, is very unlikely.
If you must fly, I recommend using a nasal spray…
Does Thermage improve acne scarring?
Thermage is not the best for acne scarring, but Fraxel
Nov 11, 2009
Thermage uses radiofrequency to generate heat. This causes tightening of the collagen fibers of the skin but also stimulates generation of new collagen fibers. The result is skin tightening. Acne scars are permanent ‘divots’ in the skin.
I have had my best results using a Fraxel laser. This is a laser that is considered fractional therapy. It has replaced the carbon dioxide laser allowing patients to carry on with their personal and professional lives 24-36 hours after treatment. I have…
Non surgical cosmetic treatments – what works?
Thermage is a non surgical alternative
Nov 11, 2009
I have been performing Thermage radiofrequency treatments to the face, neck, arms, abdomen, buttocks, and inner thighs with great results. The key is determining which patients are good candidates and establishing appropriate expectations. Patients with severe sun damage, smokers, or poor diets, are not good candidates. Since 2003, I have had significant results combining multiple modalties: Thermage and Fraxel combination, Thermage and Botox/Restylane therapy, or Fraxel with Botox and…
Will eye bags go away after excessive Botox?
Botox has a temporary efftect but will vary with each patient
Nov 11, 2009
I have several patients that experience a prolonged effect from Botox. The longest time span is about 6 months. Relaxation of the muscle of the lower eyelid usually leads to a droopy eyelid. There is a connective tissue barrier, the orbital septum, that keeps the eyefat from bulging. There is a muscular layer between the eyelid skin and the septum and that is what the Botox can relax. It is possible that with relaxation of the muscular sling, you lose that second barrier preventing the…
Who can inject Botox?
Botox is administered by a physician or nurse practitioner
Nov 7, 2009
As a board certified Facial Plastic Surgeon, I administer all injectables at my medical spa. It is crucial that the injector understand the facial anatomy and how to manage any problems that may occur. I believe that injecting Botox is an ART and having trained specifically in facial plastic surgery provides the confidence patients look for.
