The average time depends on the choice of technique for reconstruction. I always tell my patients that breast reconstruction is a process rather than a procedure. It really varies from patient to patient how much discomfort they experience with tissue expanders. Most patients find that the initial expansions are uncomfortable but that this decreases over time. Recently some reconstruction can be done over the muscle with tissue expanders (so called pre-pectoral reconstruction) and those patents seem to have less pain. As with all reconstructive options there are trade-offs to each approach.

It is always a challenge to match the radiated side to the non-radiated side. Generally if the implant is higher and tighter one can perform a release of the capsule and I have found that the addition of alloderm or any other (ADM) can help provide coverage and relax the internal capsule. It is unclear what you mean by weight. There is no body weight limit for the placement of implants. The determination of the size of the implant (that is how many cc the implant is) is based on an anatomical factors such as the width of the breast, the amount of breast tissue, stretch of the breast tissue and sometimes the nipple position. These factors can impact the selection of the implant and what size is chosen.

It is difficult to diagnose your problem without a physical exam. That said, it sounds like you could have capsular contracture this is when the capsule tightens over time and can lead to pain and discomfort. You would likely benefit from seeing a plastic surgeon who can examine you and offer a solution to your problem.

Textured implants are used when the implant is placed below the gland but on top of the muscle. They are also used on all of the shaped anatomic implants to try and prevent rotation and malposition. Smooth implants are used commonly under the muscle although they can be used over the muscle as well. I am not aware of any compelling data that one should use textured under the muscle. 
What happens if you don’t wear a binder after a TT?

All surgeons have their own specific post-operative routines, I have not found that a binder has any effect on the final post-operative results. We have the patient wear it for comfort. Most patients like to use it for 3 to 4 weeks. In theory it might help prevent seromas but I don’t think anyone has ever proven that.

Scar management is always a question that patients ask about. The only good data that exists that i am aware of is realted to the use of silicon based products on the scar. This could be a silicon scar gel or a silicon bandage. Starting after 2 weeks. You also have to be fairly religious in the use of silicon for it to have it’s full effect. Scar massage at 2 weeks is also commonly recommended by all plastic surgeons. It is challenging to make a diagnosis from the photographs but some of the drainage may be related to the blisters visible in the photographs. However, with the majority of the steri-strips on its hard to assess the enterity of the incision. I would have your surgeon look at it to decide if anything else needs to be done.

It is difficult to provide any specific answers with a photograph but there are many different kind of implants and profits (classic profile to moderate plus to high to ultra high). The implants can be round or shaped. Recently the implant companies have begun to put the more cohesive gel (gummy bear) into round implants. The implant you decribe is currently available. From your photographs it does not appear the you have symmastia. The effects your are experiencing could be related to the width of the implants. Generally pain and stretching feeling will resolve as the implants drop into their final position. Some surgeons advocate taping or the use of bands. You may want to ask your surgeon about these options. If that does not help you may want to consider switching to a higher profile implant where you could maintain the size but have a more narrow base…

I know of no evidence nor have I seen this happen with patients sleeping on their stomach with implants. I have observed flattening of the ribs when under a tissue expander but even this is relatively minor and not visible through the skin. Hope that this answer helps.

Based on the photographs alone it would appear that you have a longer distance from the fold to the nipple on the left. The would lead to extra skin and the appearance that that the implant is lower. You may need a revision on the left side at some point to achieve better symmetry. That said, there is always going to be asymmetries between the two breasts as they are not identical.
