As long as you are having this operation in a safe environment, with a qualified (board certified) plastic surgeon, you should be ok. If you have an insignificant hernia or muscle weakness, you can theoretically skip this part of the procedure. If you have very little fat, you do not need to remove this with liposuction. If your skin is not very redundant, you may get away with a smaller incision. You need to talk to your surgeon and see what makes sense for you....

Based on your picture, I doubt that you will get the look you are looking for. Your breasts are sitting low on your chest. Large implants will fill you up, but you upper chest is not going to fill up. You almost need a breast lift for getting that "fake breast" look that you are talking about. Without a picture it would be hard to tell. Generally speaking a C cup size is what most patients shoot for. The size of implant will depend on several factors including your baseline breast tissue, you height, you chest width/lenght, quality of skin ect. I think the best way for you find out what you need (short of going to see a plastic surgeon) is to look at many before and after photos. It seems to me that you want to know the difference between doing an operation (Tummy tuck) with an umbilical floating skin excsion vs. no skin removal at all (only repairing a loose muscle). If so, I would say your recovery is essentially going to be the same. Because in order to do the muscle repair you still need a incision made in the skin. The question is to remove some skin at the time or not. If you really don't need much skin removed you may be able to get by with a minor skin...
