Doctor’s answers about Rhinoplasty

Rhinoplasty to rotate tip of nose downwards?
Rotating the tip downward in Asian rhinoplasty
Dec 10, 2014
Asian patients seeking rhinoplasty often desire a more narrow, refined, and projected tip. The tip may also be turned up or overrotated such that nostril show on the frontal view is excessive. Tip grafting can turn down or counterrotate the nose and reduce nostril visibility.
How long does an Asian rhinoplasty last that requires autologous cartilage graft?
Durability of grafts in Asian rhinoplasty
Dec 10, 2014
A large volume of grafting material is often required in Asian rhinoplasty to raise the bridge and refine the tip. Enough septal cartilage is rarely available and rib grafting is often used. There is virtually no resorption of rib cartilage over time so it is essentially permanent.
After removal of the implant and graft, will I be able to push my nose up and side-to-side? Wi
Implant removal after Asian rhinoplasty
Dec 10, 2014
The goal of most Asian rhinoplasties is to raise the bridge and narrow/refine the tip. This is an “augmentation” rhinoplasty where the nasal framework is left intact and built upon with synthetic implants or grafts. Although after removal it will take time for the stretched nasal skin to accommodate to the nasal skeleton, removal should eventually restore the nose back to its original pliability.

Alar base reduction in Asian rhinoplasty
Dec 7, 2014
Many Asian rhinoplasty patients request a higher, more narrow bridge and a more narrow, refined, and projected tip. Achieving a tip size that is more narrow and balanced with a more narrow bridge often requires alar base reduction. I prefer to wait to the very end of the case before considering a base reduction. If the base appears too wide relative to the tip, I perform a conservative excision preferably using internal incisions (rather than external incision in the alar…
Who Should I Go to in NY for a Revision Rhinoplasty?
Revision Rhinoplasty in New York
Dec 7, 2014
Revision rhinoplasty is arguably the most difficult procedure in all of plastic surgery. Many noses requiring revision are both functionally and aesthetically compromised, and it is critical that a rhinoplasty surgeon be comfortable with fixing both. Complex issues such as an overrotated tip with excessive nostril show require extensive grafting to lower or counterrotate the tip and lower the nostrils. Ear cartilage is helpful but often too weak for many advanced techniques…
Can you put arnica gel on nose 4 weeks after surgery for swelling?
Swelling after revision rhinoplasty
Dec 7, 2014
Postoperative swelling is present after every revision rhinoplasty. The amount and duration of the swelling varies according to the extent of the surgery (e.g., the amount of dissection, the volume of grafts, etc.), the overall change in the size of the nose, and the thickness of the skin. Noses made larger or with thin skin will often remain swollen for less time; noses made smaller or with thick skin will remain swollen longer. Swelling does not decrease uniformly so…
How can I have a my nose proportional and balances out well with the rest of my face? What cou
Proportions in rhinoplasty
Dec 7, 2014
Proportions are a critical aspect of rhinoplasty. The proportion of tip width and projection to bridge width and height is more important to a natural, balanced nose than overall size. A bridge that is too narrow will make a proper size tip appear large. An overprojected tip will make a proper height bridge appear low. An overprojected, hanging tip can make a nose appear too long. Deprojecting and slightly rotating the tip will make the nose appear balanced. If the the…
What Is The Proper Way For Me To Clean The Dried Mucus Out Of My Nose 3 Weeks Post-Op?
Cleaning your nose after rhinoplasty
Dec 7, 2014
After a rhinoplasty, mucus can be rinsed from the nose using nasal saline mist or saline irrigations (Neti pot or sinus rinse bottle). Dried mucus or bloody crusts within the nostrils can be gently broken up and removed with a cotton swab soaked in hydrogen peroxide. It is important not to blow the nose the first week after a rhinoplasty to minimize the risk of a nosebleed. After the first postoperative week, patients can begin blowing their nose gently and can continue using…
