Without an in person evaluation of your abdomen it is difficult to say for sure if you truly need the repair. That being said I would usually recommend it as part of your surgery to try and achieve the best possible contour. While it can lead to an increase in post operative pain it is usually worth the extra effort.

From your pictures you have grade 2 breast ptosis. It is all related to the position of the nipple in relation to the fold beneath the breast and the position of the nipple on the breast tissue. Grade 2 ptosis is defined as the nipple being below the fold of the breast but not at the lowest point of the breast. That would be grade 3.

Unfortunately the answer is yes. If the graft is not adequately secured into position or if there is collapse of the underlying support structures you can have a classic open roof deformity. The good news is that it is repairable. If you feel that you have had such a complication I would strongly recommend you seek a consultation with a board certified plastic surgeon who specializes in revision rhinoplasty work.

From the looks of your pictures that is already a nice result. You are certainly early in the recovery phase following your surgery. I personally tell all of my patients that they will start to get an idea of what the final result will look like one month after surgery and even then it will continue to improve slightly. The fact that you had a very nice result after your initial liposuction is encouraging. While liposuction can remove a significant amount of unwanted fat it does not remove all of it. The use of Zoloft or other SSRI’s may lead to a small increased risk of bleeding but for the vast majority of patients this is insignificant. It can be more of a concern if the patient also has low platelets, a platelet disorder or other coagulopathy. If you do not have one of these conditions and are not taking other antiplatelet medications I would not let this concern keep you from your planned surgery.
