The hump can be removed without altering the tip. You would only need to alter the tip if you felt you needed some slight additional definition. You are a great candidate for a closed approach and from what I can tell, it would not be necessary or benefit you to use the open approach. Doing it through the closed approach should mean less swelling and recovery. Having said that, it will still take about a year to see the final settling of the hump removal! I agree with your doctor. Perfection is an unrealistic expectation and it is often impossible to gain perfect symmetry. Continuing to attempt to gain perfection is a set up for a disaster!

A facelift or mini-facelift helps reposition sagging tissue. If you don't have sagging you would not see improvement with a facelift. However, a facelift will not restore a loss of volume. Facial fillers or sometimes fat transfer may help when hollow areas develop.

A facelift may help when sagging is not relieved by rest. Permanent facial sagging may occur as early as the 30s, but more usually in one's 40s, 50s, or 60s. Everyone's genetics are different and surgery is still the best way to reset the hands of time!

Almost every human has facial asymmetries. This is absolutely normal. However, some asymmetries may be improved depending upon the surrounding anatomy. Absolute perfect symmetry is not a realistic expectation.

Alterations can be made. However, one needs a thorough consultation to understand what can and cannot be accomplished with a rhinoplasty. It would help to have a consultation with an experienced facial plastic surgeon.

From your photos it does appear there is some cheek flattening that might be enhanced. I believe in trying long lasting fillers or fat in some cases...before considering cheek implants. Cheek implants can help some people, but some of them become conspicuous as we age if a person continues to lose natural cheek volume around the implant. Some patients benefit from both implants and fillers. No matter, one should have a consultation with a specialist before relying on internet information alone.
