In my opinion, the two most important surgical procedures would be upper blepharoplasty and face/neck lift. The blepharoplasty would help you show off those blue eyes by eliminating the excess heaviness that is hooding them. The facelift is the lasting solution to the lower face and neck. I prefer a deep plane face and neck lift as I think it is a powerful and lasting approach to the aging face and neck.

I prefer to wait a full year before attempting resurfacing procedures on patients who have taken Accutane. Perhaps I am overly cautious. Around 9-10 months, a few test spots with the CO2 at different settings could determine if you are a good candidate. Radiofrequency microneedling could be a safer option if hyperpigmentation is a concern.
Is a drainless facelift more dangerous?

The purpose of drains is to provide suction between the skin and the deeper layers. This leaves little area for blood to accumulate and also promotes any small bleeds from becoming larger. There are tissue "glues" that accomplish the same thing by effectively holding the skin to the deeper structures via adhesive properties. Neither has been shown to be more effective at preventing hematomas or bruising. My preference is for fibrin glues during surgery.

A facelift will not correct this problem. Nor does it appear that you are ready for that procedure. There are easier solutions like fillers and skin tightening devices. I recommend several trials of radiofrequency microneedling followed by some fillers several months later if necessary.

A chin implant would be very helpful for your profile. An additional benefit is that it elongates your jawline, making the neck look more contoured. Discuss with the surgeon of your choice which implant will work best for you. Some fill in the pre jowel area but can also leave a square look if your lower face is already wide. It is impossible to tell which is best for you without good anterior pictures.

The biggest unknown is whether you have enough skin/SMAS laxity to remove all of the widened scar tissue. One of the best determinants is a good physical exam that assesses how well the tissue moves where the deep plane would be developed. It sounds as though the deep plane approach will be necessary to correct these problems.

Fractional CO2 can gratly improve the appearance, however results depend on the strength of the settings used. And your skin dictates how strong the setting can be. It will take multiple treatments regardless. RF Microneedling is similar in its ability to help acne scars but requires more treatments than fractional CO2. Usually, people do this involuntarily when taking pictures as they subconsciously feel the weight of their brows holding their lids down. You would be a candidate for upper and lower lid surgery. Insurance will only cover functional eyelid surgery, and this appears to be more cosmetic. I would approach your lower lids through the inside of the eyelid (transconjunctival) and use the lower lid fat to fill in the tear trough area. Alternatively, if you didn't want surgery, a chemical brow and eyelift with neuromdulators to the crows feet and forehead would open the eyes more. Based on your pictures, you seem like an excellent candidate. The incisions should be hidden in your sideburn and creases of the ear. These will be barely visible as long as they are not closed under tension. That is a huge benefit of deep plane techniques. The incisions behind your ear should be inspected at every visit after surgery. Products with vitamin E and silicone can greatly hasten the recovery process.

It appears from the picture you posted that your left brow is lower causing the eye to look slightly more closed or smaller. That could be a habitual behavior, an undesired outcome of neuromodulators like botox, or your anatomy. A good physical exam would be able to determine the cause and the treatment options. Surgical options like a brow lift can improve asymmetries but will not fix them 100%. Neuromdulatrs and fillers in targeted areas can also help improve the symmetry and avoid surgery.
