Cephalic trim of lower cartilages involves removing the upper part of the lower cartilages and essentially getting rid of it, unless the surgeon used it as grafts in another part of the nose.
A 5-0 nylon suture is a permanent suture, so in order to reverse it, it will have to be cut out and removed.

It is tough to predict how a 14 pound weight loss will change the outcome in a particular patient without a proper examination of skin quality, breast shape, etc. There is definitely a possibility that weight loss can alter the outcome from previous surgery. The main thing to keep in mind with nose filler is that because it is adding volume to your nose, along with potentially providing further definition to the nose, it will also make the nose bigger (augmentation). This is a great question and it is ultimately up to you and how badly you desire the breast surgery.
During pregnancy, the breasts engorge with milk, and post-pregnancy depending on how much time is spent breast-feeding, the breasts will remain engorged. And then once breast-feeding is finished, the breasts will deflate as milk production ceases. Everyone’s skin quality is different, and it is difficult to predict how well the skin will shrink alongside the breasts.

From the photo, it appears that there is now an open wound which appears inflammed as evidenced by the redness. The wound will heal from the inside, out. It is difficult to diagnose mild-moderate cases of capsular contracture from a photo, unless it is causing deformation of the breast. The hardness you are feeling may be the scar tissue that is around areolar region from the mastopexy. It can also be from the scar formation around the open incision on the left breast. It sounds like you are pretty fresh postop (1-2 weeks) if your columella stitches are just being removed. If so, the columellar incision/swelling has a lot of time to take for it to heal. Post-operative swelling in an open rhinoplasty, which you had, can take up to a year for it to go down, with the tip of the nose taking the longest on that spectrum. The columella scar can also take up to 1 year to remodel and heal completely.

Cephalic trim of the lower lateral cartilages is a common maneuver in rhinoplasty. A common misconception is that the lower lateral cartilages lay right at the nostril rim at the edge. On the contrary, the lower lateral cartilages actually angle upward away from the nostril rim.

The darkening of the nipple/areola is a phenomenon associated with pregnancy and occurs as the body prepares itself for breastfeeding. The darker nature of the nipple/areola helps the newborn in locating the nipple for breastfeeding. The dark color returns to the normal pink color as breastfeeding ceases. So to answer your question, the nipple/areolar color will unlikely return to the darker color unless you become pregnant again.

1) 19-21cm is what is used as a general guideline for the measurement between the sternal notch to nipple. While that may be a range, that measurement can be tweeked to tailor to each patient. While the philtrum is situated over the orbicularis oris muscle, the philtral width is not determined by the amount of facial exercise, and will unlikely change by facial exercise. The philtral widening you are experiencing may be attributed to postoperative swelling or maneuvers performed during the rhinoplasty. Consider visiting a rhinoplasty surgeon for a proper examination.
