Septoplasty surgery and sinus – insurance or no insurance?
Functional Nasal and Sinus Surgery Combined? Yes
May 2, 2019
Thanks for your question. Typically corrective functional nasal airway surgery, i.e. Septoplasty, Turbinate Reduction, Repair of Vestibular Stenosis, are often combined with Endoscopic Sinus Surgery within the same operation. Additionally, these surgical services are generally covered by most major insurance carriers. You may ask your surgeon and/or hospital to pre-operatively obtain a pre-certification of coverage to be sure. This will also generate an estimate of “out of pocket” expenses …
Can I remove my internal splint after rhinoplasty by myself?
Removal of Internal Nasal Splints Yourself? NO!
May 1, 2019
It would not be recommended for you to attempt removal of your internal nasal splints yourself. You could damage your nose, cause unnecessary bleeding and create an infection by attempting this. Best to find a local US physician (Facial Plastic, Plastic or ENT) to evaluate you in their clinic and remove them for you. Best Regards,
My columella is gone and I feel I look terrible. Will it loosen up over time?
Correction for Flat Appearance to Columella after Septorhinoplasty? Yes
May 1, 2019
Thanks for your thoughtful questions and posted photos. I am sorry that you are experiencing these concerns after nasal surgery. I share your concerns. The shortening of the septum, strut graft or tongue-in-groove techniques may have been involved with your original surgery, which resulted in your understandable concern now. The tightness and stiffness you are experiencing, and even facial pain,  is typical post op and will certainty improve over time. Whether or not this loosening eff…
How long does it take for the tooth numbness to go away after endoscopic sinus surgery (FESS
Numbness in Central Teeth after FESS
Apr 8, 2019
Thanks for your question. Did you also have a septoplasty along with the FESS procedure? During a septoplasty, the greater palatine nerves may be affected, and this results in temporary alteration in sensation to the upper teeth. While common, this is often an annoying concern of patients. Fortunately, the sensation usually returns to normal within the first 9 month. If you are beginning to feel some sensation or can detect that your teeth are “waking up”, these are good signs. 3 months ma…
Would a chin implant improve my aesthetic looks along with a tip plasty?
Rhinoplasty and Chin Considerations
Apr 8, 2019
I am glad to read about your thoughtful quest for research regarding enhancements to your nose and chin. Your pictures reflect some lovely facial features and you seem to have a naturally beautiful nose and chin. I would respectfully suggest you proceed with caution and meet with several experienced specialist for true and honest opinions about your concerns. Study actual patient results during your consultations.  While having surgery may offer you some rhinoplasty and chin enhan…
I noticed a tiny hole on my septum the size of a pen nib. Does a hole that size repair itsel
Micro-Nasal Septal Perforations
Apr 1, 2019
From your description, “the size of a dot mark made by a thin pen”, you may have a micro-nasal septal perforation. I am sorry this happened to you. Unfortunately, even these tiny pin hole perforations will typically not heal themselves. If you keep this area clean, free from scabs infection, the opening should “stabilize and heal” permitting your surgeon to consider repair at a later time. Tiny holes are generally easier to correct, but you should have a discussion with your surgeon abo…
How to fix a protruding caudal septum?
Revision Rhinoplasty Considerations
Apr 1, 2019
Thanks for your thoughtful questions regarding excess columellar length, tip support and tip ptosis while smiling resulting in the appearance of a shortened upper lip. A review of your previous operative notes, photos and nasal schematics, along with a complete in-person examination would be necessary to completely answer your questions. That said, it appears that despite a primary rhinoplasty, your septal length, dorsal projection, tip projection and tip support may benefit from a revision…

Chin implant Infection Complications
Mar 26, 2019
I am sorry to read about your complications and infection after placement of your chin implant. It would be best to have this chin implant removed and wait at least 6 months before considering a replacement. If your infected implant was originally placed through an intra-oral incision, you may wish to consider an external incision next time. If your implant is truly infected, then you should have it removed now. Best wishes.
Should I see a plastic surgeon or an Ophthalmologist for my ptosis?
Eyelid Evaluation from a Specialist
Mar 26, 2019
There are many qualified and highly regarded surgeons in specialty practices devoted to eyelid and ptosis patients. You may wish to consult with an Oculoplastic Specialist with specific interest in eyelid and ptosis evaluations. Depending on your physicians’ training and experience, you may also find surgeons with Plastic, Facial Plastic or Ophthalmology expertise who may offer sound advice and best recommendations. Searching for a highly qualified specialist, with whom you feel co…
Should I wait for a blepharoplasty at 39 years old?
Upper Eyelid Considerations
Mar 26, 2019
Thanks for your post. In my view, eyelid surgery may be the best option to clean up the excess skin and enhance overall eyelid appearance. A conservative, well planned and performed upper eyelid blepharoplasty from a trusted surgeon should make you happy. Personally, if you are truly bothered by this now, I would encourage you to move forward this year. Best Regards,
Is there a solution to getting rid of unsightly capillaries 1 year post-op?
Treatment for Nasal Capillaries and Spider Veins
Mar 25, 2019
This is a common concern for some post operative nasal patients and rosacea cases. I have found the best therapy is with the KTP laser, which can essentially make them disappear by selective photocoagulation. The treatment can be performed in the office by physicians or surgeons without anesthesia, and typically takes 15-20 minutes. The physician traces over the vessel with the laser until the desired effect is seen. Other laser systems may be effective as well. The best systems utilize a la…

Smaller Nose via Revision Rhinoplasty? Maybe
Mar 20, 2019
Thanks for sharing your post and photos regarding your interest in a revision rhinoplasty to achieve a smaller nose and tip. I would gather your rhinoplasty records, photos, operative reports and nasal schematics related to your primary procedure and share this information with your chosen revision rhinoplasty specialist. By having an in-person examination and understanding what was originally done to your nose, how it looked before any surgery, etc., this may assist a surgeon determine th…

Nasal Obstruction and Drainage
Mar 19, 2019
Persistent symptoms of nasal obstruction, drainage, rhinitis and recurring infections may require further workup. Rhinomanometry or acoustic rhinomanometry are tests that can measure nasal airflow. This may not help with cases of nasal valve collapse, which can be seen with professional examination in the office. You may also benefit from nasal cultures, diagnostic nasal endoscopy, allergy testing and a review of your CT sinus studies. These studies may suggest other medical or surgical tr…

Revision Rhinoplasty Considerations
Mar 18, 2019
I am glad that you are devoting the effort to have confidence in your chosen revision rhinoplasty specialist. It is well worth your time! I personally find that there is no best substitute for an in person consultation and evaluation for a revision rhinoplasty. Chose someone with years of experience and a proven, photo patient profile record for natural and improved results. You case appears rather straight forward and may not require additional rib cartilage. Cases that require rib or dono…
Will a revision be successful?
Revision Rhinoplasty Concerns
Mar 4, 2019
I am sorry that your primary rhinoplasty did not give you the masculine, symmetric and improved breathing that you had hoped. I share your concerns. Fortunately, you may be an excellent candidate to consider a revision rhinoplasty with placement of cartilage grafts via an open approach. You still appear to have a dorsal hump, ptotic tip, long septum with columellar show, narrow middle vault and wide nasal bone base. You could consider improving the columellar length by shortening of the s…
