Can my nose be corrected further through open rhinoplasty?
The answer is diagnosis and technique–not open or closed

It’s impossible to tell anything from one photograph, though your nose seems narrow, your tip is pinched, your nostrils are too arched, and your airway must be compromised. The solution is not to choose the approach, but rather what is done. I only do closed rhinoplasty for a reason: the skin determines half of the shape of the nose and the open approach makes it impossible to assess the skin and to perform certain maneuvers. Some surgeons and patients have the idea that they can “see b…

Your Lower Nose can be improved in another 6 to 12 months if you wish

I do not see your columella as wide from the inferior view, which is good because that is a largely uncorrectable complication of open rhinoplasty that cannot be repaired. However, it is low as your nostril contours are irregular. These, and the scars, are common problems after open rhinoplasty, which is why I never perform it. Without seeing the surface, the surgeon has no validity to fine-tune the nasal shape and must rely on sutures and struts, any of which create new problems. You ar…
Nasal valve collapse following open primary rhinoplasty
Rhinoplasty May Commonly Inadvertently Compromise the Nasal Valves

The internal valves (the sidewall in the middle third) depend on the height and integrity of the bridge. When the bridge is reduced even 2 mm, the internal valves become unstable and can collapse and decrease the airway by 50%. I know this by my published 600 patient rhinomanometric study over 16 years. The external valves (the sidewall in the lower third) depend on the position and strength of the tip cartilages. If the lateral crura (the tip cartilages above your nostrils) are reduced, or i…
How to proceed with a possible revision rhinoplasty following open rhinoplasty 14 months
A change in dorsal height is very disconcerting for many patients, especially men.

In 2012 I wrote a paper entitled, “What Motivates Secondary Rhinoplasty?” In it I reviewed 150 consecutive secondary patients to see why they wanted to undergo another operation. Some had new deformities. Some had uncorrected problems. But 15% had undergone an intolerable change in ethnic, personal, or familial characteristics. I even wrote a section about loss of identity in my textbook.I speculate that that is part of why you can’t adjust to the new nose. You originally had a …
I’d like to know the least invasive way to build my bridge and tip back up after rhinopla
The least invasive and most protective surgery is always closed rhinoplasty

In terms of surgical access and risks to the patient, there is no comparison between closed and open rhinoplasty. In the closed approach, I never uncover areas that are not directly related to what the patient asks me to do. This limits recovery, adds precision, and minimizes the chance of introducing new deformities. I think the worst thing a surgeon can do is add problems that the patient never had. That’s in a marked distinction to open rhinoplasty, where the tissues are routinely di…
Nasal Valve Collapse? Or something else? What has happened to my nose post 7 months rhino
Collapsed internal valves look like an “Inverted-V” and decrease the airway by 50 percent

You have very smartly made the right diagnosis. When the bridge is reduced, that resection takes off the cartilaginous roof, which means that the sides of the nose in the middle third (the internal valves) no longer have the support that they did, and will fall toward the midline, obstructing the airway and making the middle of the nose seem narrow compared to the bony part.

What to do about a huge bump on the side of my nose that appeared 1 year post-op. It’s is
It’s not the bump – – it’s the dent below it that counts because this is your airway compromise
Jan 25, 2018
Jan 25, 2018
If you look at your preoperative photographs, your right nasal bone is more prominent than your left. After the surgeon reduced the bump, he or she inadvertently decreased support to the middle third of the nose on the side – – this is a critical area call the internal valve. Because the bump is not there, there is nothing to hold the sides out in the middle third, so they fall toward the septum in the center. In doing so, they make the right nasal bone look even more prominent– the bone ha…
Can radix graft be reduced and hump shaved in a closed simple rhinoplasty without additio
Grafts are easy to revise through closed rhinoplasty

One of the advantages of closed rhinoplasty is that only a few grafts are ever necessary – – I only use radix, spreader, tip, and alar wall grafts to fix almost every nose.Because the surgery is closed, I never need internal sutures, columellar struts, alar rim grafts, soft triangle grafts, or any of the techniques that are used to repair the damage caused by the open approach itself.

Nose became less defined as it healed –1 month – 6 months, is this excess scar tissue? (
Nasal skin determines half of the result; that’s why closed rhinoplasty is so successful

One of the fallacies upon which rhinoplasty has always been built is that if the surgeon makes a perfect skeleton, the skin will take on that shape. Unfortunately, it’s not that simple. Skin has its own vectors of contraction – – posteriorly and medially (backward and toward the midline) in the upper nose and concentrically and backward around the tip – – those are the vectors that create supratip deformity and make the tip look round and shapeless.
