What are your opinions on my strange hooked-like swelling after surgery?
post op swelling
Feb 19, 2019
It looks like post op swelling in the supra tip area. this is pretty common as it is an area that gets manipulated a lot during surgery. it sounds like you have a good doctor who is providing you with careful and thorough follow up. thats the most important part.  I don’t personally use accutane.  For your type of localized persistent swelling I would inject a small amount of steroid (triamcinolone).  This can be done anytime within the first few months after surgery and may have to be re…
Can I do Botox the same day if I’m a ballet teacher?
botox and exercise
Feb 19, 2019
The standard and typically conservative recommendation after Botox treatment is to avoid alcohol and strenuous exercise until the next day and avoid laying flat or inverting the head for 4-6 hours. it sounds like your ballet class is not that strenuous to teach and you should be fine. But if your concerned you could always get a later appointment after class.
What non-invasive procedures are recommended after a face and neck lift?
noninvasive procedures after facelift
Feb 19, 2019
Great question.  There are a several products out there to help maintain skin tightness and stimulate collagen production.  They can be ultrasound (such as ulthera) or radio frequency (such as excillis) based. Additionally there may be textural or pigmentation issues you may want to address.  Depending on your skin type you may be a good candidate for laser resurfacing or peels.  Finally maintenance with good skin care (high spf titanium or zinc based sun block) Retin A and vitamin C as we…
How many vials of Kybella would I need to achieve good results?
how much kybella
Feb 19, 2019
This is a difficult question to answer without an in person consultation.  Most patients with a similar profile photo as yours can achieve a satisfactory result after 2-3 treatment sessions spaced 6 weeks apart.  I charge my patients by the session not the vial.  the exact amount of product needed depends on the surface area being treated and that can only be determined during an in person consultation.  Most patients need between 1-2 vials per treatment.  Hope this helps.
Are tear trough and lower eyelids considered separate areas or one in the same?
difference between tear trough and lower eyelid
Feb 19, 2019
The lower eyelid is an anatomical structure that extends from the lash line to the lower lid cheek junction.  The tear trough describes a deformity that can occur in the lower eyelid with aging that appears as a shadow or indentation along the medial (side closer to the nose) portion of the lower eyelid.  Tear trough deformity can be addressed with injection of fillers such as restylane.  Other issues can occur in the lower eyelids such as lower lid bags above the tear trough and fes…
Crooked Nose or Uneven Swelling After Rhinoplasty?
asymmetry 1 week after rhinoplasty
Feb 19, 2019
It is not unusual to have some asymmetry when the cast is removed 1 week after rhinoplasty.  There is usually a lot of swelling and asymmetric swelling is quite common and can be the cause of what you are noticing.  I recommend regular follow up with your surgeon.  The final result from your rhinoplasty won’t be visible until 6-12 months after surgery.  If there is still unsatisfactory asymmetry at that point, you should speak with your surgeon about a possible revision.
Non-surgical Nose Job Procedures to Reduce Nose Size?
nonsurgical rhinoplasty
Feb 19, 2019
Nonsurgical rhinoplasty refers to the use of fillers to create the illusion of straighter lines in the nose and create an overall smoother appearance in the nose.  This means the addition of products to the bridge to smooth out an uneven appearing hump or help camouflage asymmetries.  Nonsurgical rhinoplasty cannot reduce the size of the nose.  Looking at your photo, another option without undergoing rhinoplasty is chin augmentation which can be done surgically or with fillers as your pr…
How To Treat Hole In Nose From Cocaine Use?
How To Treat Hole In Nose From Cocaine Use?
Feb 19, 2019
Good news is this can be repaired.  First of all congratulations on getting clean.  A hole in the septum can be repaired surgically or filled with a silicone septal button.  How yours should be addressed depends on the location and size of the septal perforation and how much it bothers you and how much you are willing to go through.  A septal perforation at the very front of the septum can result in loss of support of the tip of your nose and in those cases I recommend surgical repair with…
How does a nose job hold up after 10 years? What about a broken nose?
How does a nose job hold up after 10 years?
Feb 19, 2019
This is a very good but also complicated and loaded question.  First I would like to challenge the idea that the nose or the septum tries to redeviate or go back to its previous shape.  If a rhinoplasty or septoplasty is done well, this should not be the case.  However there are many different techniques to perform a rhinoplasty and every surgeon approaches the nose slightly differently.  You are right to be concerned about the appearance of the nose 10 or 20 years after surgery.  If the …
Shouldn’t I have had better results for so much money?
Shouldn’t I have had better results for so much money?
Feb 19, 2019
It looks like you are 4 days out after injection of a neuromodulator.  The units can be tricky depending on which product was used.  Typically for the areas you described I use about 50 units of botox which is the equivalent of about 150 units of dysport.  I am not sure which product you had.  Depending where you are the cost may differ but what you paid is definitely in the ballpark for treatment of the three areas you described.  As far as the result, I do see a softening of all the lines …
Facelift vs filler? 25 years old
face lift vs fillers at 25
Feb 19, 2019
Based on your young age and your photos, it looks like you have had some significant volume loss in the face with your weight loss but no excess skin.  At 25 your skin has great elasticity and does not appear to have significant laxity.  I would recommend replacing the volume loss with fillers or fat grafting (which can be more permanent).  I’d recommend juvederm or volure in your nasolabial folds and the corners of your mouth, restylane in the tear troughs and depending on your aesthetic pr…
Which procedure gives optimal outcome for my eyes as received different opinions from surgeons? (Pho
which procedure for lower eyelid midface contour
Nov 19, 2018
It is not surprising that you have been given different recommendations for the same problem by different doctors.  The main reason is that this is a difficulty area to treat and continues to be an area of academic debate among facial plastic surgeons, plastic surgeons and oculoplastic surgeons.  It seems that all your consultants have given you reasonable recommendations and discussed possible risks with some of the procedures.  Essentially what looks like is going on with your lower lid…
Is an overall nose reduction possible and realistic?
nasal reduction
Oct 30, 2018
Thank you for your question.  You would be a very good candidate for rhinoplasty.  An overall reduction of your nose is definitely achievable.  Based on your photos I would recommend osteotomies to narrow the dorm, and refinement of the tip cartilage as well to refine the bulbous tip.  It is definitely possible to achieve a natural non “Michael Jackson” look.  In fact with thicker skin as you have, the struggle would be more likely that during the initial post operative period there may be m…
Rhinoplasty. Would I need cartilage taken from another part of my body?
rhinoplasty questions
Oct 30, 2018
Thank you for your questions.  For a standard rhinoplasty, I would recommend taken 1-2 weeks off from work.  In your case it is likely that your surgeon would perform osteotomies to narrow the bridge as well as use grafts to build up the height of the bridge.  Different surgeons will have different preferences for material, some prefer cartilage, others prefer synthetic material such as Gortex.  If you have never had prior nasal or septal surgery, there may be enough cartilage in your sep…
2 weeks post-op. Is the hump going to go away?
dorsal hump 2 weeks post op
Oct 30, 2018
It is very normal to have swelling in the initial weeks after surgery.  In your case looking at your photos it looks like there is significant soft tissue swelling under the skin of your nose which is within the range of normal post operative swelling.  This is definitely expected during the post operative course.  I tell all my patients that the swelling can last a few months even up to 1 year.  I recommend you see your surgeon to discuss your specific concerns.  Not being in the oper…
