Can ENT Doctor fix blockage instead of going to another surgery again? Almost 1 year post op on Tip an
Blockage after rhinoplasty
Mar 22, 2017
Unfortunately, I feel that the only way to truly determine your cause of obstruction is from a good physical exam. There are so many possible etiologies that looking at a picture of the outside of your nose will not be of much help. I encourage you to find a good ENT, facial plastic surgeon in your area and get an evaluation. In your case several recommendations may be in order before deciding on the best option.
How long to wait for a 3rd neck lift revision?
Scarring after facelift
Mar 22, 2017
I am terribly sorry that you have such issues after a facelift. Unfortunately, you will have to wait a bit for the inflammation to calm down. Ideally it would be at least six months from your initial surgery. This is because currently there is a lot of blood flow to the area. Lifting the skin up will create more bleeding than the average patient. This increases your risk of developing a hematoma and thus complications from surgery.
Nose broken when I was a toddler. Is it possible to straighten it?
Straightening the nose
Mar 22, 2017
Your nose can definitely be straightened. In general i tell my patients that I will get them 90% straight, the last 10% is how they manage the nose after surgery. It is true that the nasal bones tend to try to shift back to their original position especially in long standing fractures. This is because your left nasal bone is shorter than the right due to the extreme deviation. In general I try to not add grafts which tend to widen the nose as most patients do not like a stronger, broader...
Is my nose really crooked or is it just swelling causing it to look that way? What would be best to fi
nasal trauma
Mar 22, 2017
First off, it is very difficult to assess the results of surgery at 10 days. The nose typically swells up 1/3rd greater in size. Over then next 6 weeks much of this swelling will resolve. It almost never comes down symmetrically and will create a shifting nose. At 6 weeks around 80% of the swelling will be resolved. Keep following up with your surgeon. I often have my patients massage their nose to keep it straight as the swelling is resolving. Your surgeon will be able to better inform you...
Is my septum severely deviated? What's wrong with my nose?
Septal deviation
Mar 22, 2017
The end of your septum may be jutting out slightly into the left nostril. If this is not causing you blockage in your breathing then it does not need to be addressed. Often this "spur" sticking into your nostril can be from trauma, underlying asymmetry to the bone, or from a long septum that "grows" into the nostril as you are developing. If there are no symptoms to it, I recommend you ignore it as only small children and big dogs will be looking at that view of your nostrils.
Devastated Nose after Septoplasty?
Deviated septum
Mar 22, 2017
The short answer is that you do not need another septoplasty if your chronic sinusitis is well managed. The CT scan images are not exactly at the same level, so there may have been some underlying deviation prior to surgery. Unfortunately, however, I have seen patients come in with new deviations after prior septoplasty. Often this comes from leaving inadequate support to the end of the septum that is palpable when you put your finger in your nose. If you decide to seek treatment for your...
Turbinate reduction and septoplasty 11 days ago - is the disposable packing is still there or not?
nasal blockage
Mar 16, 2017
Unfortunately without seeing you it is impossible to answer your question. What I can tell you is that dissolvable packing takes time to go away. Nasal saline sprays and gentle irrigations help it dissolve faster. when you see your doctor, he may be able to suction out some of the excess packing to improve your breathing faster. As to the crookedness of your nose, the typical septoplasty does not involve straightening the nose. It takes care of the cartilage on the inside that you can't see...
Acne scars: I've tried a ton of things - thinking of facelift because they all go away when stretched.
facelift for acne scars
Mar 16, 2017
I would recommend that you do not try a facelift for acne scars. I am currently treating a patient that did the same and she spent a lot of money without improvements. Your skin is thick and youthful. You likely do not have laxity. I would recommend that you try an aggressive fractionated CO2 laser treatment or find a dermatologist who is comfortable with phenol for ice pick scars. To improve your scarring is often a multimodal therapy as the volume has to be replaced and the thinned...
What happened to my sideburns after facelift?
Sideburns after a facelift
Mar 16, 2017
The location of your sideburns depends upon your plastic surgeon. He may have made an incision that goes in front of your ear and into your hairline, this tends to rotate the sideburn back and up. It looks like from the photos he may have made a high incision on your sideburns, this has the effect of raising them up. If you have a lot of facial hair, you can grow sideburns to cover up the incisions. I tend to make a low sideburn incision for this exact complaint. Most people do not want...
Lines like indentations or dents 3 weeks after fractional CO2 treatment. No improvement over 2 weeks,
CO2 laser
Mar 16, 2017
When I have seen issues such as your it often is from a deep pass that delivers a lot of energy to the skin. You are likely seeing the space between their treatment squares. I have attempted to alleviate this by always doing two passes so that every millimeter or skin gets treated with the laser. At this point, microneedling with PRP, IPL or other laser treatments that promote healing or just waiting or your best options. I would discuss your concerns with your doctor as they will be...
Can you have filler for under the eyes and upper lip at the same time as having dot CO2 laser done?
filler with lasering
Mar 16, 2017
Thank you for your question. I frequently perform fat grafting and fractional CO2 laser treatments the same day as it minimizes down time as both cause significant swelling. I tend to inject fillers either before treatment or same day with the one caveat that I am careful not to overfill as this will lead to excessive swelling. I try to avoid injecting after lasering as I notice that patients are swollen for up to 4-6 weeks after treatment. When it comes to sensitive areas like the...
Fractional CO2 laser on face, is it normal to look like this 2 months after treatment?
CO2 laser results
Mar 16, 2017
Thank you for the photos. They help immensely in determining the next course of action. Your doctor likely did one pass over your face or at least one deep pass. On that pass, there was too much energy given and now you have inflammation and redness. If it is just red streaks and lines where the end of each pass was, this will typically slowly improve as the skin heals, there may be some scarring requiring further treatment. if the skin looks rough, you may need further treatment to...
Is there a procedure or surgery that could fix the symmetry of my eyes/eyelids?
Eyelid asymmetry
Nov 14, 2016
You have two options- surgery or volume replacement. Surgery is difficult- it can be very hard to anticipate exactly where the folds will occur and how much fat to remove from your right side. Also you have to be careful that your surgeon does not remove too much skin and make your eyes look "done," or worse yet leave you unable to close your eyes completely. Safer, is to try revolumization of the left subbrow fat pad. This will fill out the skin and make it match the...
Is it normal to still have significant swelling 1 year post-op (rhinoplasty)?
Swelling at 11 months
Nov 14, 2016
Typically the worst of the swelling has resolved by 6 months. It is true that a small amount of swelling can persist even up to a year and a half after surgery. Typically the swelling decreases each month a small amount. At 11 months, you might get a little more definition of your cartilages but I would assume that your overall profile is going to be stable. That being said, with endonasal rhinoplasties, it is quite common for the tip to drop over time as many of the ligaments and soft...
Am I a good candidate for a nose job and chin implant?
Surgery
Nov 14, 2016
Yes, you are a terrific candidate for surgery. I recommend seeing a couple of different surgeons, asking questions and making sure that you agree with their vision for your nose.
