Dr. Bhrany received his medical degree from Boston University School of Medicine then in Massachusetts. He later completed his residency in otolaryngology (ENT) – head and neck surgery at the University of Washington (UW) in Seattle. He was then awarded fellowships in Head and Neck Surgery and Facial Plastic Surgery at the University of Auckland in New Zealand, and UW, respectively. During those two years of additional sub-specialty training, Dr. Bhrany developed skills for cosmetic facial plastic surgery, reconstructing all aspects of the head and neck, as well as head and neck cancer surgery and microsurgical reconstruction.

He has been a faculty member of the UW since 2008 and trains other surgeons in Facial Plastic Surgery, both at the UW School of Medicine, and as an invited speaker around the country. As long as your overall health is good, you can certainly be a candidate to undergo a facelift. I personally have safely performed a number of facelift surgeries on patients your age or older. At the time of consultation, your surgeon should review your goals and desires in the context of your health to create a plan that works for you and optimizes your outcome.

Your question is a good one. As you alluded, facial fillers can be composed of different materials including hyaluronic acid (ie Juvederm, Restylane), calcium hydroxyapatite (Radiesse), and poly-L-lactic acid (Sculptra)- each with their advantages and disadvantages for use. It would be very unusual for headphones use to result in significant soft tissue atrophy to result in such and indentation without you feeling the pressure while wearing them. While this specific question has not been scientifically studied, as an otolaryngologist and facial plastic surgeon who treats patients with sinusitis, undergoing cheek filler injection is not contraindicated in patients with chronic sinusitis as long as your chronic sinusitis is under control. You are very early in the post-operative period from liposuction, and it is not uncommon for their to be some mild irregularities (swellings, dents) in the areas treated that will be present for a weeks to months as swelling resolves. From the photo you provided, it appears that you would be a great candidate for a facelift and neck lift to address the jowls and neck laxity. Whether or not any face or neck surgery will worsen your melasma is a good question. Melasma is typically not worsened after undergoing a face and neck lift. But to help prevent a flare up, it may be useful to prepare your skin with topical treatment prior to undergoing the surgical procedure.
