A concavity, or saddling, of the middle third of the nose defines the so-called saddle nose deformity. The bridge can be restored to any height, and a hump can certainly be added. The goal of most Asian rhinoplasties is to raise the bridge and narrow/refine the tip. This is an “augmentation” rhinoplasty where the nasal framework is left intact and built upon with synthetic implants or grafts. Although after removal it will take time for the stretched nasal skin to accommodate to the nasal skeleton, removal should eventually restore the nose back to its original pliability.

Narrowing the nasal bones may slightly increase the height of the bridge but it will not lengthen the nose. Lengthening the nose is difficult and requires grafting to pull the tip down and hold it there as the nasal skin contracts during the healing process. In revision cases where the septal cartilage is depleted, rib cartilage is often necessary to lenghthen the septum and rigidly fixate the tip in a more appropriate position.

Overaggressive excision of septal cartilage during a septoplasty can weaken the nose and lead to both functional and cosmetic complications. If too much cartilage is excised from the most forward, or caudal, portion of the septum, the nasal tip will begin to droop. If too much cartilage is removed from the central portion of the septum, the bridge will begin to collapse and create a saddle nose. Correction requires rebuilding the septum with cartilage grafts.
