The problem scar is atypical for elective neck platysmaplasty. It is not clear from your description what was the original issue and why such a scar was required. None surgical options are limited in this area but can be tried. Ultimatly scar revision with immediate steroids and posthealing steroids may be most effective

A good rhinoplasty is an operation of subtle refinement that should never look like a nose that was operated upon. Most of such operations can be performed under local awake anesthesia for those patients that desire such option. Modern general anesthesia is relatively safe and should not be feared. Either option is available to most patients. Local anesthesia with sedation often has less postoperative bruising and less postoperative nausea.

From the foto this appears to be a depression in the supra tip area were the lateral crura of the lower lateral cartilage expand.Fillers would create a distorted nose and more bulbous tip. Consult an experienced rhinoplasty surgeon and I suspect from the picture there is permanent fix for your concern

Your concern is a valid one as liposuction alone will yield a minimal benefit and I gather you desire a more substatial change. I found in my experience that combining neck liposuction with a “wattle neck” procedure that is an internal tightening suspension of the underlying musculature will yield additional benefit. For scars such as one you are showing an office performed dermabrasion can be quite helpful. This not to be confused with microdermabrasion which is much more superficial and has no real effect on scars. The dermabrasion can be performed in the office and serves to smooth out the transition boundary between normal tissue and scar itself. Fractional and ablative lasers can offer similar benefits but not to the same degree and not at same cost as dermabrasion.
