An ENT/ facial plastic surgeon would be a good start.
Also important to evaluate the internal portion of the nose to rule out a deviated septum, since the nose is so crooked.

I think your chances of getting a significant improvement in the appearance of your nose are quite high but look for a plastic surgeon who is very experienced in rhinoplasty.
The nose can be reduced in overall size and the crooked appearance reduced to create a very natural appearing profile that fits your face.

Fixing a Large Crooked Nose Intelligently Requires UNderstanding Its Complexity and Appropriate Planning
While your complaints are common, they need to be handled with care. Your bony vault and cartilagenous vault are disjuncted or noncontiguous.
This makes the rhinoplasty slightly more complex. The osteotomies (breaks in your nasal bone have to be done very carefully and your dorsal aesthetic lines need to be restored in addition to removal of bobe and cartilage from your dorsum.
You also need your septum straightened. This needs to be done by a surgeon well versed in reconstructive and aesthetic procedures of the nose and face.
If I were to perform such a rhinoplasty, I would plan on taking a small part of one of your rib’s cartilages to have strong and straight material to perform an aesthetically sound and lasting rhinoplasty.
It may be possible to complete the rhinoplasty with cartilage removed from your nose but this must be determined intraoperatively. In my opinion you should only consider Surgeons who have completed a true plastic and reconstructive surgery training followed by a craniofacial fellowship and handle all kinds of revisionary and reconstructive rhinoplasties.
There are all kinds of practitioners out there that perform rhinoplasties but in general I believe you are in the best hands with a plastic and reconstructive surgeon who has extensive craniofacial surgery training and experience.

You are absolutely a candidate for surgery. My advice would be first to find a surgeon in your area that is certified with the American Board of Plastic Surgery. If you go online they will have a listing of certified doctors in your area. Set up a consultation and meet with the doctor to iron out a surgical plan that will address all the issues and help to achieve the results that you are looking for.
Ask to see before and after pictures of patients who have undergone similar procedures. Remember to keep in mind that no two patients are exactly the same and results do vary, however, this should give you a gauge of the type of work the surgeon performs. Should you not feel comfortable at any time you are free to seek another doctors advice. I always advise patients that they have to be comfortable with both the doctor and the office staff because you will be following up with the office for quite sometime. You have a hump on your bridge and your nose is crooked. These can be fixed with a closed approach and you should have a very nice outcome. In general, patients with your nasal appearance can get a significant cosmetic improvement with an artistically performed rhinoplasty.
