Simple answer: not in this lifetime. It appears you have relatively thick skin of your nose, especially in the tip. When this is the case, even overcompensating while modifying your framework won't show externally because of your skin texture. Secondary skin resurfacing could be disastrous if that was attempted soon after a revision rhinoplasty in an attempt to improve skin texture. I have always drained these, no matter how "dry" they look at the end of surgery. I have always been impressed by how much drainage there is and how it tends not to start for a few days and may continue for weeks, even though I have always been light on the cautery during surgery so had less damage to the remaining tissues. The risk of a seroma becoming a secondary infection far outweighs any risk of infection from a drain. And by the way, capsule thickness has no correlation with toughness....

As far as postoperative swelling, you should have peaked 3-5 days after surgery, so that stretching already happened. What you have by now will still remodel, but the size won't really change much. Change out any time; it sounds as if you didn't get what you asked for. Speak up to your surgeon, who may not realize your disdain. Did you pick the size or were you told what size to get without your input??

The wrinkling should be considerably improved with some soft tissue fill such as fat transfer, but not excessively so it does not look puffy and full (other than the first 1-2 weeks). If your FIRST picture was your abdomen prior to the tummy tuck, I would have suggested a muscle wall repair with lipo of the abdomen to take care of your muscle spread after the twins and stimulate some skin tightening while also thinning your silhouette. After reviewing your pictures, it appears you have a few problems here that time will not cure. The low scar has significant asymmetry in length from side to side while your midline depression ("front butt") points in the opposite direction as it goes "south." If your diastasic was "corrected," it appears this was either insufficient or not performed based on your rectus muscle bulges in your pictures. Based on your pictures, it looks most probably like your nasal bones spread after surgery and not callus. Any cutting or fracture of your nasal bones in surgery will first form a scar along the fracture/osteotomy line, not make the whole bone be prominent. It could be soft tissue swelling over the bones from the dissection in surgery, but that usually doesn't present like this or at this time after surgery. You'll see what your surgeon says......

It appears your problem is upper fullness and insufficient lower pocket, not sagging. When the sub muscular pocket is tight at the bottom, the implant will bulge in the upper pocket. Think of a see-saw on the playground with a 300-lb person on the other end from you. You need to selectively stretch the lower pockets with pectorals muscle stretches and compression exercises on the upper half of the implants. Keep at it because exercises may resolve this while surgery...

I have done transaxillary approach since 1987 and have often seen this as an early postop finding. It has usually been corrected via exercises instead of more surgery. It appears the lower pocket may not quite be stretched out enough to allow the implants to "drop," which is usually NOT due to implant migration. You need to stretch your lower muscles.
